A Message from the Executive Director
Dear Friends,
Did you know CILO provides Home & Community Transition Services? These services assist people with disabilities who are currently residing in rehabilitation facilities, hospitals, nursing homes, or other institutional settings with their transition back into the community to live as independently as possible. CILO can assist the consumer to:
- Identify the healthcare, social services, and daily living support needed to live independently
- Connect with organizations and agencies that can provide ongoing services and support
- Apply for disability benefits
- Find affordable and accessible housing
- Obtain home modifications
- Obtain disability-related equipment and devices
- Other needs that promote independent living
The position of Home & Community Transition Coordinator was an ideal fit for our longtime employee Amanda Hardcorn, MSW. She brings vast experience working with people with disabilities through facilitating the HOME Choice Program and then the Kentucky Personal Care Attendant Program. Her coordination of these transition services for consumers in both Ohio and Kentucky will assist people with disabilities to lead independent and inclusive lives in the community.
