Independence at Home Demonstration Shows CMS Savings

The recent Centers for Medicare & Medicaid Services’ Independence at Home (IAH) Demonstration results showed that the project has consistently saved CMS millions of dollars since launching in 2012. IAH was designed to test the effectiveness of employing comprehensive home-based primary care services and sought to reward providers who deliver quality care while reducing costs.

 

In 2020, year seven of the demonstration, approximately 5,000 beneficiaries were cared for by 10 practices. These participating practices saw a change in savings of about $459 per beneficiary per month. The estimated reduction in Medicare spending exceeded incentive payments to IAH practices by $4.2 million.

In March, Age-Friendly Health Systems, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA), will start its next Action Community, a seven-month virtual learning opportunity on adopting the 4Ms (i.e., what matters, medication, mentation, and mobility) with a network of teams from across different health systems. It is designed as an “on-ramp” for hospital-based teams (e.g., emergency departments, intensive care units, general wards, medical-surgical units), ambulatory care teams (e.g., primary care, specialty care), nursing home teams (e.g., post-acute and long-term care), and convenient care clinic teams to test and adopt age-friendly care.
