QUALITY AND ANALYTICS CO-LEAD

The Middlesex London Ontario Health Team (MLOHT) is seeking an individual with quality improvement, evaluation, and analytics experience to be the Co-Lead for our Quality and Analytics Advisory Group. We are seeking an in-kind resource from a partner organization for this role.


The MLOHT Quality & Analytics Advisory Group will work to develop and implement an evaluation strategy to ensure that we are appropriately measuring the advancement of our purpose.

ONTARIO HEALTH TEAMS: THE PATH FORWARD WEBINAR

The Honourable Sylvia Jones, Deputy Premier and Minister of Health, provided an important update last week on the next steps that the ministry and Ontario Health will take to accelerate OHT impact. Deputy Minister Dr. Catherine Zahn, Ontario Health President and CEO Matthew Anderson, and Betty-Lou Kristy, Chair of the Minister’s Patient and Family Advisory Council also shared their reflections.

COMMUNITY HOMELESSNESS SUMMIT

The Middlesex London Ontario Health Team took part in the Health and Homelessness summit in early November. Following an in-depth conversation at the Coordinating Council, it was established that the MLOHT would get involved in tackling the homelessness crisis. Homelessness may be associated with physical health issues, addiction challenges, mental health challenges, and these are challenges that require a system-wide and whole of community response.

LOCAL RESEARCH EXPLORES HOW TO BEST MANAGE CARE FOR OLDER ADULTS

Does your work involve the coordination of care for older adults? We'd like to speak with you! The research team is gathering feedback about care management and coordination across the system, to try to improve integration between primary care and home care for older adult patients, their caregivers and providers. Participation will involve a short interview (less than 30 minutes) with the research team. If you are interested in participation, please contact the principal investigator, Dr. Jacobi Elliott Jacobi.elliott@sjhc.london.on.ca.


Reimbursement for your time will be provided. 

JOB OPENING – PROJECT ASSISTANT

We are looking for a dynamic individual who is passionate about healthcare system improvement to work effectively with us, our members, and stakeholders in the role of Project Assistant. The individual will provide administrative and project support to the Clinical Lead (Primary Care), Clinical Lead (Specialty Care), the London Middlesex Primary Care Transformation Lead and various provider partners.

Remote Care Monitoring


We've been approved for funding to support Remote Care Monitoring to enhance a patient's quality of life following their surgery.

  • Through the Middlesex Hospital Alliance, $46,000 will be used to support 170 hip and knee total joint replacements with Remote Care Monitoring (RCM).  
  • Another $80,000 has been approved to support 200 thoracic surgeries with RCM through London Health Sciences Centre.
  • $79,646 has been approved for Home and Community Care Support Services to expand the Telehomecare Program for 50 additional patients.


Let's go home


We are happy to announce that Cheshire London received confirmation that the Let's Go Home Implementation Plan has been approved and the program went live this week.


Let's Go Home supports providers and patients by removing non-clinical barriers to discharge/community stabilization (For example, legislated co-pay for CSS services, coordination between multiple CSS providers, alignment of clinical & community supports).

EXPRESSION OF INTEREST - LONDON’S COMMUNITY SAFETY AND WELLBEING PLAN (CSWB) 

The Community Safety and Well-Being (CSWB) Plan is looking to recruit 2 to 3 new members for the CSWB Advisory Committee. CSWB is a provincially mandated initiative that supports working collectively towards a community where everyone is safe, has a sense of belonging, and can meet their needs for education, health care, food, housing, income, and social and cultural expression. The role of the Advisory Committee is to guide and implement the overall vision and strategy of the CSWB Plan.


Interested applicants are asked to respond to the questions in the application detailing their qualifications for the role. Completed applications are to be sent to Alexis Kampman at akampman@london.ca by Friday January 13th, 2023.

IMPROVING HEALTH EQUITY FOR THE FRANCOPHONE POPULATION

We are working with the Erie-St.Clair/Southwest French Language Health Services Planning Entity (Entité 1) to strengthen French language health care services with our Francophone population. 


Health care organizations interested in enhancing their French language services are encouraged to email us or visit the Entite 1 website.


The entity offers advice to OHT partners to ensure compliance with the French Language Services Act to meet the needs of the Francophone population.


This includes identifying Francophone patients, gaps in French language services and expanding partnerships.

We want to thank Dr. Scott Laing for his contributions to our Operations Team as the Masters of Health Administration Intern and his ongoing leadership in our Referral Management work.  


Dr. Laing is leaving our region to resettle in the Kitchener-Waterloo area where he will be working with the Centre for Family Medicine and eHealth Centre of Excellence. His input was instrumental in creating the eReferrals forms that will be used provide-wide. We wish him all the best in his future endeavours!  

ONTARIO STRUCTURED PSYCHOTHERAPY PROGRAM

Referrals will be accepted starting December 12 to the Ontario Structured Psychotherapy Program to help people better manage depression and anxiety-related conditions with short-term evidence-based cognitive behavioural therapy (CBT).
