Clackamas County Health Centers (CCHC) and Project Access NOW are partnering to improve community health, thanks to a grant from Clackamas County Public Health. This collaboration supports Alicia, a Community Health Worker (CHW), who connects patients at the Sunnyside and Beavercreek clinics with vital resources.
"Our partnership with embedded PANOW Community Health Workers has allowed us to expand the screening of our patients for Social Determinants of Health and to provide direct outreach and connections to countless community resources in Clackamas County. We couldn’t do this work without them!” says Leslie King, Primary Care Operations Manager at CCHC.
Focusing on high-priority populations, often those with diabetes, mental health challenges, or other chronic conditions, Alicia conducts social determinants of health screenings. Patients, with CHW support, identify needs like food and transportation assistance. Alicia then links them with resources, including those within PANOW, and helps them navigate OHP benefits. One client shared, "My eviction has been canceled...Thank you so much for your part in making this happen!"
Healthier Oregon and the 1115 Waiver (HRSN), while expanding access, have also created confusion. Many are unaware of their options and rights, and fear can prevent them from seeking help. This partnership empowers community members through education and resource connection.
