Home Modification Training

The Home Modification team completed their final quarterly training for this calendar year. Case Managers are reminded that all Home Modification forms, current trainings, and presentations are on the Home Modifications website. Please note that Community Centered Boards and Single-Entry Points have separate trainings. 

Case  Management Redesign Home and Community Base Rule Revision Stakeholder Engagement  


The Department will update rules and regulations pertaining to Home and Community-Based Services (HCBS) waivers and non-waiver programs, to come into alignment with new Case Management Redesign (CMRD) statute, policy and language.


Rule updates are required to clean up areas in existing rule and waivers that are repetitious, obsolete, or difficult to follow and understand. CMRD also implements changes to the rule structure. The vision behind the structure change is to make the HCBS rules easier to find, read and understand for all stakeholders. Rules must be in effect no later than Nov. 1, 2023, at which point the first cohort of new Case Management Agencies under CMRD will transition.

Here is a list of our upcoming meeting dates and topics through November. Please note these dates are subject to change.

Reminder to Community Centered Boards

The Department has identified a large-scale issue regarding Risk Factors for members on both the HCBS-DD and HCBS-SLS waivers. Case Managers need to ensure that all these waiver members have both an initial and then a subsequent annual review of risk. Members should be supported to discuss their risk factors with their Interdisciplinary Team annually. Case Management Agencies will then need to document the member’s Risk in the Bridge. Case Management Agencies have been given a report in SharePoint to specifically identify members who need these corrections.

Community-Based Organization Ambassador Program

The Colorado Department of Health Care Policy & Financing (Department), which administers Health First Colorado (Colorado’s Medicaid Program), is working with Colorado communities to start an Ambassador Program. The goal of the Ambassador Program is to work with Health First Colorado members and people who might be eligible for services to improve the letters and other written information the Department uses to communicate with Coloradans about Health First Colorado. The Department is focusing first on Spanish- and Vietnamese-speaking communities (this announcement is available in Spanish and Vietnamese here).


Ambassadors are people who will help us understand the needs of the communities they live in. They will partner with community organizations that work with Health First Colorado members to help make the information members and people who may be eligible get easy to understand in their native languages and in culturally responsive ways. 


Ambassadors will help Health First Colorado with: 

  • Leading interviews, focus groups, and listening sessions 
  • Getting feedback about the best ways to communicate with members
  • and potential members
  • Sharing guidance for policies and programs
  • Providing important information to Health First Colorado members and
  • potential members


Ambassadors will be paid an hourly wage and get training and support from the Department.


We are asking for trusted community organizations to contact our subcontractor, Health Management Associates (HMA), if they are interested in supporting the Ambassador Program. This could include hosting an Ambassador or providing information about potential Ambassadors in their community. The program will be run by the Department’s Member Experience team.
