The CalAIM Provider Forum is designed to give current and prospective providers of Enhanced Care Management (ECM) and Community Supports services the opportunity to share knowledge, ask questions, and collaboratively problem solve.

Join your local CalAIM community in September for an in-person PATH CPI Collaborative meeting at Sierra Health Foundation in Sacramento. This event is an opportunity to collaborate with providers and stakeholders on key issues like Community Supports needs, implementation challenges, and best practices. Have you been experiencing challenges with transferring Medi-Cal memberships between counties? 


Homebase wants to hear from homeless service providers in Sacramento CoC who are struggling to help their clients transfer their Medi-Cal membership from county to county. Homebase is working with the National Health Law Program (NHeLP) and Western Center on Law and Poverty (WCLP) to address this problem and identify specific cases in which people experiencing homelessness have not been able to carry out an inter-county transfer of their Medi-Cal coverage from another county to Sacramento. Barriers to inter-county transfers can prevent people experiencing homelessness from accessing health care and other benefits, securing medication, and finding housing. 


We encourage you to email Medi-Cal@homebaseccc.org if this problem applies to your agency and you have a case to share. Homebase will then connect you with NHeLP and WCLP so they can learn about the scope of the problem. Please do not email Homebase any specific details of a case or clients' identifiable information.

Submit Letters of Interest for New Funding Opportunity: Advancing the Community Health Worker, Promotor, and Community Health Representative Workforce in Medi-Cal 


The California Health Care Foundation is seeking letters of interest for a new funding opportunity aimed at strengthening the Community Health Worker, Promotor, and Community Health Representative (CHW/P/R) workforce in Medi-Cal. This initiative will provide grants, mentorship, technical assistance, and tactical tools to Medi-Cal providers. Providers interested in shaping the initiative's design and funding levels are encouraged to submit their letters of interest.


Letters of interest are due August 23, 2024.

DHCS' Enhanced Care Management and Community Supports Quarterly Implementation Report Now Available! 


The Department of Health Care Services (DHCS) has released the Enhanced Care Management (ECM) and Community Supports Quarterly Implementation Report, covering data from January 2022 through December 2023. This report offers a detailed overview of the statewide implementation of ECM and Community Supports, key components of California’s efforts to transform Medi-Cal into a more coordinated, person-centered, and equitable care system. The report includes state, county, and plan-level data on member enrollment, utilization, and provider networks, reflecting submissions from managed care plans to DHCS.
