MASKS ARE REQUIRED BY LAW



To Our Patients,

We are a healthcare facility. By law, California medical offices and hospitals require masks to be worn. We look forward to when this is no longer the case.

CURRENTLY - MASKS ARE REQUIRED BY LAW.

Dozens of patients a day are showing up without masks.

WE NO LONGER PROVIDE MASKS TO PATIENTS OR GUESTS.

You will not be allowed entry to our office without a mask.

This is for the safety of our staff and our patients.

Masks can be purchased across the street at Cal Oaks Pharmacy

If this makes you late for your appointment you may have to reschedule.

FOWH MASK AND VISITOR GUIDELINES

1. Everyone must wear a mask at all times in the office.
2. Please bring masks. We do not provide masks.
3. You will not be allowed entry to our office without a mask.
4. Masks can be purchased at Cal-Oaks Pharmacy across the street.
5. NST's (non-stress tests) are in a shared room so we still have to restrict visitors (except when needed for support or assistance).
6. For all other appointments, ONE visitor (one adult or child) is allowed.
7. We are NOT asking patients or visitors if they are vaccinated for COVID.
8. Our safety requirements include mandatory mask-wearing for patients, visitors and staff; use of hand sanitizers; waiting room spacing (using our text message system); and others.
9. Please do not come to the office if you have symptoms of possible COVID or ANY OTHER CONTAGIOUS ILLNESS such as the Flu. Sore throat, cough, allergy symptoms, stuffy nose, all can be COVID or the FLU. These are highly contagious infections.
10. We have flu vaccine available.
