Welcome back to another edition of the Home Visiting Central Intake Bulletin! This email will provide you with Help Me Grow Central Intake data and updates relevant to Home Visiting (HV) providers.

SFY22 Fourth Quarter Referral Data

(April- June 2022)

Central Intake and Referral (CIR) processed 11,321 system referrals in the the fourth quarter. A referral breakdown is below:

  • Caregivers (9.4%);
  • HEA8045 forms (8.7%) submitted by HV providers, as well as WIC offices, and other community agencies;
  • Medical professionals (11.7%);
  • Non-medical community (57.9%) including WIC, community agencies, county agencies, and electronic Pregnancy Risk Assessment Form (e-PRAF) referrals; and
  • Public Children Service Agency (PCSA) 8021 forms (12.3%) submitted by children’s services agencies.


Of the 11,321 system referrals, about 58 percent of those who accepted Home Visiting were pregnant women, and 43 percent were first-time moms.

CIR continues to make referrals to the Pathway HUBs, which have totaled 1,093 from September 2021- June 2022.

Home Visiting Family Profile Videos

CIR worked with video production company Pixel Planet Studios throughout SFY22 to film three vignette videos featuring families enrolled in Home Visiting. Please use the videos below to educate and promote the impact of Home Visiting services.

Marketing and Outreach Reminders

Visit the Refer-A-Friend webpage

The Refer-a-Friend Giveaway launched this summer as an incentive to encourage current and past Home Visiting participants to refer families they know to the program. Outreach staff delivered posters to Home Visiting providers in MIECHV counties to promote the giveaway, and it has been advertised on social media.

Outreach staff delivered promotional cards to Home Visiting providers that encourage current families to share their Home Visiting stories on the helpmegrow.org website. 


Please give these to the families you serve so they know how to share their positive experiences and successes. Their stories could be shared on the Help Me Grow website and in other promotional materials.

2022 Ohio State Fair

In case you missed it, CIR, in collaboration with the Dolly Parton's Imagination Library of Ohio, hosted a Help Me Grow booth at the 2022 Ohio State Fair this past summer. Booth activities included craft, selfie, referral, and story time stations where families gathered to sign up and learn more about Help Me Grow services and the Imagination Library.


An estimated 7,200 fairgoers stopped by the booth over the 12 days of the fair! CIR saw an increase in family and friend referrals during the fair's duration for both Home Visiting and Early Intervention.


We would like to thank our collaborators at the Governor's Office; leadership at the Ohio Department of Health and the Ohio Department of Developmental Disabilities; outreach and intake staff statewide who worked the booth; and First Lady Fran DeWine for stopping by to read a book during story time.

ASQ Online now available

If you haven't heard, CIR is now offering the Ages & Stages Questionnaire (ASQ) Online screening!


Ohio families and anyone working with children under age three can use the screening to understand how a child is developing. Once the screening is completed, someone from CIR will contact the family to discuss the results.


The ASQ Online serves as a child-find strategy for Help Me Grow services. Since its launch in the spring, CIR has been able to refer families who used the screening tool to both Home Visiting and Early Intervention.
