Growing up multi-racial as 2nd generation Chinese- American and 3rd generation
Mexican-American, Jade knew that she was her ancestors dreams. Her family was very involved in the United Farm Worker (UFW) Movement in the Gilroy, Salinas and Watsonville area. Her great grandfather, Antionio Del Buono, was a labor organizer and farmworker and her grandmother, Cecilia Weymouth, was an educator and organizer. At a young age, service and volunteerism were instilled in her. Now she hopes to continue this legacy with her own daughter in her own community.

Jade (she/her) has a Bachelor's degree in Women's Studies from UC Santa Cruz and a near-complete Master's degree in Ethnic Studies. After founding a non-profit called fundakid, which ensured all youth access to high-quality programs and educational experiences, she worked with Boys & Girls Clubs of Sonoma-Marin, serving children and their families in Northern Sonoma County. There, Jade led a 20-person team overseeing 500+ youth, three school districts, and several stakeholders and community members. In tandem, she earned her Master's in Organization Development from Sonoma State. As a founding member of La Familia Sana, Jade came on board as the Executive Director of La Familia Sana in 2021.
There, she works to implement the organization's core values of grassroots advocacy, empowerment, and equity throughout the organization and northern Sonoma County community.

Sonoma Connect | Sonoma Unidos will be holding a year-end celebration and holiday mixer. The event will incorporate activities in place of the Community Accountability Action Team with time to get to know each other in person. Light snacks and refreshments will be available.

Free Diversity, Equity, and Belonging Workshop! | |
Sonoma Connect is offering a FREE Diversity, Equity, and Belonging (DEB) workshop by Equity First Consulting to our Community Accountability Action Team members and network partners.
The workshop series will support the creation of common language and understanding for teams, organizations, and community to better grasp the system of oppression and how it manifests itself within organizations. It will also support our understanding how to operationalize DEIB-based practices, center equity, and design to the margins to transform systems of care in our community.
The series of four workshops will be offered January 11th & 25th and February 8th & 22nd from 3pm-5pm. Participation in all 4 trainings is required to ensure the full benefit of the learning opportunity.

CalAIM Office Hours: How Organizations Are Sharing Data for Enhanced Care Management (ECM) and Community Supports
On December 1, from 2 to 3 p.m., DHCS will virtually host an “Office Hours” discussion (advance registration required) on how CalAIM organizations are sharing data for ECM and Community Supports. Participants are invited to submit questions by November 28 to CalAIMECMILOS@dhcs.ca.gov. Attendees will also have an opportunity to ask questions during the session.
