CIE PARTNER MEETING RECAP

Hello CIE Partners,


Thank you for attending the CIE partner gathering on April 25, 2024. Your participation and enthusiasm truly made the event a memorable and successful one.



During the meeting, the participants shared what they would like to be known/ remembered for and the lessons they wanted to pass down to the group. One person expressed their desire to be remembered for their infectious laughter, ability to see the unseen, hear the unheard, and serve the unserved. A participant quoted Maya Angelou, emphasizing the importance of making people feel valued and remembered and we ended the activity with a powerful reminder to make peace with our past and prioritize our mental and physical health, so we can better care for others in the future.



Partner Highlight


Christy Rosenberg from the San Diego Wellness Collaborative shared details about their work with the Gravity Project Payments & Reports Discovery Workgroup and how the San Diego Wellness Collaborative is using CIE for its CalAim Care coordination efforts. 



We learned about Christy's dedication and collaboration with healthcare teams, partnerships with healthcare organizations, work and engagement with community stakeholders, and forward-thinking approach to transforming the future of healthcare. 



We are grateful to have Christy as a pillar in our community. 



New Direct Referrals


We learned about two new direct referrals offered by Serene Health currently available to clients with an active Medi-cal managed health care plan from Blue Shield Promise, Molina Healthcare, Kaiser, and Community Health Group currently enrolled in the Enhanced Care Manaement plan. Elizabeth Rincon from our Partner Experience team conducted a demo on how to "Authorize Clients in CIE." Elizabeth not only explained the steps but also provided an in-depth overview of the resources available to support partners through the process. If you missed the demo, don't worry! CIE Team Updates



Zenia Torres shared an update on the important work with the Justice-Impacted Affinity Group, a collaboration between CIE, the Asian Solidarity Collective, Second Chance, Neighborhood House Association, Metro Community Ministries, Inc., San Diego Workforce Partnership, Public Safety Group, and Sheriff's Department. 



Together, they are working to identify challenges and barriers from both a provider and client/lived experience perspectives, and share their

 recommendations via the Justice-Impacted Collective Community (Care) Plan. This J.I. Collective Community (care) Plan will be shared out with the network very soon so, stay tuned!

Alana Kalinowski provided an update on the Bi-Directional Data Integration with Kaiser Permanente via the Unite Us referral platform which enables Kaiser Permanente users to send direct referrals electronically to CIE partners and receive outcomes in their EHR via the integration.


Lastly, Joanna Oboza provided a demo on how to utilize the CIE CalAIM webpage and resources that 211/CIE created to streamline and centralize CalAIM information www.211sandiego.org/cal-aim

2024 CIE Summit


Registration is Open for our 8th Annual CIE Summit!

Join us for the 8th Annual CIE Summit! This hybrid event will be hosted in person at the University of San Diego campus’ Hahn University Center. This year will kick off with a pre-conference CIE 101 session on June 24th and two full days of engaging sessions on June 25-26.

This year’s content will be focused on sharing innovations in designing and implementing community-led care infrastructure from emerging CIE models across the nation. Workshops will focus on lessons learned, best practices, and practical applications across early to late stages of planning and implementation. Cross-cutting topics will focus on:

  • Centering Lived Experience
  • Healthcare In Action
  • Aligning with Policy


Each session will spotlight tools and resources that can be brought back to your own community for application with the option to select from the following breakout tracks tailored to the different stages of building a CIE:


  • System Build Phase
  • Launch Phase
  • Scale Phase


This hybrid event offers an in-person experience in San Diego, CA, or a virtual experience, powered by our conference platform Whova.
