CHWs Leading the Way to Sustainability

Community Health Worker sustainability refers to the ability of CHW programs to continue providing effective services over the long term. Sustainability includes funding, organizational support, integration into healthcare systems, and ongoing community engagement.

As CHWs, your dedication and passion have been instrumental in improving health outcomes and fostering community empowerment. Now, it's time to take our efforts to the next level by ensuring the sustainability of CHW programs and work.

By participating in CHW sustainability efforts, you'll have the chance to

advocate for Ourselves and Our Communities:

  • Stand up for the recognition and support that CHWs deserve within healthcare systems and policymaking circles.
  • Advocate for sustainable funding and resources to ensure the longevity of CHW programs and services.

Ways to participate: Join one of our work committees above and stay connected to us for upcoming opportunities.

Read below for an upcoming opportunity.

Call for Community Health Workers to serve as Maine Care Consultants

Purpose: Maine Care is seeking Community Health Worker (CHW) consultants who will inform how CHWs are incorporated within Primary Care Plus (PCPlus) program.

PC Plus encourages primary care practices to incorporate CHWs into care teams to improve the health of their patients. The model aims to improve healthcare quality while also reducing costs. CHW consultants will be part of the engagement process to determine how best to incorporate CHWs into PC Plus reimbursement.

Eligibility: MaineCare is seeking individuals to serve as consultants that meet the following criteria:

• The individual must work in a paid or volunteer CHW position (see definition, roles, and competencies) or have 2 years of prior experience working in a CHW position.

• Permission of their supervisor/organization to serve as a consultant during work hours and/or flexibility to serve as a consultant during non-work hours.

Consultants will be compensated for their time.

Self-Care Tip

Self-Care Tip

Heart Health means so much more than eating right and exercising. Your stress plays a big role in your health. Please see the link below that provides more information on stress and how it effects your heart health.

Read more here

What is self-care? Self-care means taking the time to do things that help you live well and improve both your physical, health, and mental health. When it comes to your mental health, self-care can help you manage stress, lower your risk of illness, and increase your energy. Even small acts of self-care in your daily life can have a big impact.
