Wellness Education Benefit Case Manager Trainings

The Wellness Education Benefit (WEB) will be educational materials mailed monthly to Children’s Home and Community Based Service (CHCBS) waiver members. The WEB is defined as individualized educational materials designed to prevent hospitalization or movement into an institutional setting. The WEB helps members in obtaining, processing, and understanding information that assists with managing health-related issues, promoting community living, and achieving goals identified in their person-centered service plans. The WEB also provides members with actionable tools and information for informed decision-making and preparation for conversations with medical and social service professionals.

This benefit will be available to members in May 2024. Information surrounding the WEB will be important for case managers so they can assist in discussing the benefits of the WEB with waiver members. The WEB is an important service for members and may support continued waiver eligibility.

These mandatory CHCBS case manager trainings will be held on:

  • April 2, 2024 - 1 to 2 p.m.

  • April 4, 2024 - 10 to 11 a.m.

  • April 10, 2024 - 2 to 3 p.m.

CHCBS case managers only need to attend one of these training sessions.

All meetings will use the same Zoom link and phone information.

Older Adult Stakeholder Meeting Discontinued

HCPF first and foremost wants to say Thank You for all of your valuable input and support over the years. Your insight has been invaluable in helping to determine the best approaches and solutions to projects that positively impact older adults and individuals with intellectual and developmental disabilities. As the three ARPA projects represented in this meeting are fully operational and nearing the end of their timeline, HCPF has decided to end the Older Adult Stakeholder meeting. We do not want to lose your participation and your expertise, and we hope you will join us in other meetings, which are always on the Office of Community Living Stakeholder Engagement page.

ARPA 8.10 - 1115 Substance Use Disorder (SUD) Waiver Amendment Public Hearing

HCPF is pleased to share updates regarding our efforts to provide comprehensive medical coverage, and related expansions of our current 1115 Expanding the Substance Use Disorder (SUD) Continuum of Care Waiver (1115 SUD Waiver) that will be submitted to CMS in April 2024.

HCPF is requesting an amendment to the current 1115 SUD waiver, which would authorize:

  • Continuous eligibility for ages 0-3 and adults leaving a Colorado Department of Corrections (DOC) facility;
  • Prerelease services for individuals transitioning from correctional facilities; and
  • Reimbursement for acute inpatient and residential stays in institutes for mental disease (IMD) for individuals diagnosed with a serious mental illness (SMI) or serious emotional disturbance (SED).

The amendment is currently posted on HCPF’s website and open for public comment until Feb. 24, 2024, at 5 p.m. MST.

Members of the public can also provide comment during upcoming public hearings taking place Feb. 7 & 8. More information about the public hearings is on the website.
