FRONT DOOR LEAD AGENCY CHOSEN FOR LOCAL NAVIGATION MODEL

We are pleased to announce that Home and Community Care Support Services (HCCSS) South West has been chosen as the lead agency to provide coordination of services 7 days a week to improve access to care, and transitions for our community as part of our Local Navigation Model.


HCCSS met several criteria including meeting Ontario Health expectations and can work with other OHTs within the South West region.


A team has been created with members of HCCSS, the MLOHT, Huron-Perth OHT and Elgin OHT who are meeting weekly to design next steps.


The plan must be completed by the end of March 2023.


The plan consists of:

  1. Developing a screener/intake tool
  2. Map intake and navigation pathways/Standard Operating Procedures
  3. Develop the team
  4. Execute the model

MLOHT KEY PRESENTER AT OHT IMPACT FELLOWS KNOWLEDGE TRANSLATION SERIES

Our Impact Fellow Mulugeta Chala shared his insights on how to develop strategies for building capacity to evaluate OHT initiatives and why it's so important at an OHT Impact Fellowship Program webinar on February 22nd. Another member of our Operations Team, Matt Meyer, Population Health Lead, also presented at the event which received a great response from those in attendance.

OHT Supports Events

EREFERRAL VIA EFAX CONTINUES TO EXPAND

The project has encouraged many organizations to adopt and use eReferral within their organization.


London Health Sciences Centre is implementing eReferrals starting with the General Surgery Department. LHSC and St. Joseph's Health Care London are exploring other potential areas for expansion.


Since eFax was integrated to Ocean eReferral in May 2022 within the South West region, close to 250 referrals were sent using efax.


The project has successfully improved the ability of primary care providers to track and follow their referrals and they are recommending it to their colleagues.


We want to thank the London Middlesex Primary Care Alliance and Ontario eServices Program for collaborating in this project.

NEW REGIONAL OHT DIGITAL HEALTH STRATEGIST

In an innovative partnership, five OHT's in Southwestern Ontario have welcomed Silas Ng as the Digital Health Strategist for Middlesex London, Elgin, Sarnia-Lambton, Chatham-Kent and Windsor-Essex. This newly created role will ensure that Ontario's road map for digital health reaches its full potential locally. Silas joined us in late February and brings a wealth of experience including leading a team at the Ministry of Health to develop and implement the province's digital health strategy.

NEW CLINICAL PROJECT ASSISTANT

We are excited to announce a new addition to our team. Lindsey Moore joined us on February 27, 2023 as a Clinical Project Assistant.


Lindsey will be responsible for providing administrative and project support to the Clinical Lead (Primary Care), Clinical Lead (Specialty Care), the London Middlesex Primary Care Transformation Lead, and various provider partners that are actively engaged in Middlesex London OHT initiatives/improvements.

LET'S CELEBRATE

  • Our abstract for the International Conference on Integrated Care was accepted for a 90 minute Workshop - Using Co-Design and Engagement Principles for Health System Transformation: Our Lessons Learned
  • We transferred $20,000 to Middlesex London Health Unit to provide taxi vouchers to families in need whose children require updated vaccines to meet the school mandate. (Alliance for Healthier Communities Funds)

Our Patient/Client, Care Partner Council (PCCPC) wants to hear from you. We want to hear from patients, people with lived experiences, and caregiver tables across Middlesex London to ensure patient/client and caregiver's voices are heard.

HEART TO HEART: COMMUNICATION SKILLS ESSENTIAL IN PALLIATIVE CARE

This highly interactive, full-day workshop will allow you to practice leading sometimes difficult, but important, conversations around goals of care, decision-making, and planning. Valuable communication skills gained will be relevant to all aspects of your medical practice and encounters with patients.

PHSS FOUNDATION - HONOURING DR. CATHY FAULDS

PHSS Foundation is honouring Dr. Cathy Faulds in its 24th Annual Salute to Laudable Londoners. Dr. Faulds currently serves as a palliative care physician with St. Joseph’s Health Care Centre and is a staff family physician at London Health Sciences Centre.
