We observe National Hispanic Heritage Month by celebrating the history, culture, and contributions of American citizens whose ancestors came from Spain, Mexico, the Caribbean, and Central and South America.

Rosh Hashanah 2023 begins on Friday, September 15, 2023, and ends on the evening of Sunday, September 17, 2023
Join us in celebration of Rosh Hashanah, the Jewish New Year. According to the editors from History.com, it is one of Judaism’s holiest days. Meaning “head of the year” or “first of the year,” the festival begins on the first day of Tishrei, the seventh month of the Hebrew calendar, which falls during September or October. Rosh Hashanah commemorates the creation of the world and marks the beginning of the Days of Awe, a 10-day period of introspection and repentance that culminates in the Yom Kippur holiday, also known as the Day of Atonement. Rosh Hashanah and Yom Kippur are the two “High Holy Days” in the Jewish religion.

September National Health Observances:
Healthy Aging, Obesity Awareness, and More
Each month, the US Department of Health and Human Services selects National Health Observances (NHOs) that align with their priorities for improving health across the nation. In September, they are raising awareness about healthy aging, childhood obesity, substance use recovery, sickle cell, and HIV/AIDS.
Healthy Aging Month was started to encourage people to focus on their health and take precautions to help them face the challenges that come with aging. It started over 30 years ago in 1992 when people born in the 1940s were turning 50. It spread the message that it is possible to adopt a healthier lifestyle at any age and mature gracefully.

Ride FACTNOW
Residents of San Diego County with disabilities using a mobility device in need of a wheelchair-accessible van (WAV) can use an on-demand paratransit service for same-day transportation with as little as one hour's notice. The service costs $10 one-way only.

Individuals using a wheelchair, walker, or other moblity devices. Served San Diego County

Building Solid Ground
Addresses the housing needs of formerly incarcerated individuals who currently reside in or are re-entering San Diego County. The program will serve those released from California state prison and/or on parole within the past five years, prioritizing those recently released and including individuals with mental health issues and PC 290 offenses on a case-by-case basis.

Resource and Referral Hub
Provides information and screening for SAY San Diego programs and connects clients or caseworkers to eligible resources, including housing, parenting, health, mental health, education, employment, and income

Anyone is eligible to reach out for screening; each program has different eligibility requirements. Serves San Diego County.

Senior Solutions- CDBG
Provides free transportation for immunocompromised low-income adults (62+) residing in the City of San Diego. Transportation includes Lyft, Uber, MTS Access, and taxi vouchers for essential trips (medical, grocery shopping, pharmacy, banks...).

62+, living in the city of San Diego, immunocompromised, low income (under 30% AMI). Must live in City of San Diego (921-- zip)

WHITE PAPER: Opportunities for Enhancing Medicaid (CalAIM) Incentive Models to Center the Whole Person | |
Co-authored with Health Leads, the aim of this paper is to summarize findings from analysis of countywide data to explore with more specificity what influences social drivers of health (SDOH) hardships among people who have called 211 San Diego or other partners within the CIE. This summary describes key findings answering the following questions:
1. What kinds of hardships do people experience, and how do these vary by their identities?
2. How severe are these hardships?
3. How do community members interpret the relationships identified in the first two questions?
We conclude with some insights and potential next steps based on these conversations as a means of sharing lessons early and often with fellow practitioners and advocates across the country. We welcome any insights from the field as readers are experiencing them in contexts similar and different from the one in this paper and any questions to learn more about these analyses by emailing analytics@healthleadsusa.org.

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.
Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.
