Children attending public school in Iowa must be up to date on immunizations. You can find Cedar Falls Schools student immunization requirements online, including meningococcal and Tetanus, Diphtheria & Pertussis requirements for students entering 7th and 12th grades. These are required BEFORE the first day of school.

Schedule your child’s annual checkup – and vaccination if needed – with a MercyOne family medicine provider near you. A copy of your student's immunization record can be dropped off or mailed to the building (Attention School RN) they will attend this fall for school year.

Please help nursing staff prepare by updating your family’s records. Visit the Health Services page to find these forms and complete those which apply to your family.

  • Medication – Parent Permission
  • Self-Medication Consent
  • Allergy Action Plan
  • EpiPen Authorization and Release

Please note that some forms REQUIRE a medical provider's signature(s). Completed and signed forms can be dropped off or mailed to the building (Attention School RN) they will attend this fall for school year.
