Plan of Care (POC) Updates and FAQ

Happy Friday!


There have been some questions after the last trainings on the Plan of Care (POC) and we want to ensure we all are on the same page.

 

Marshall and DHHR are working closely with NWIC to train providers on the Wraparound Model. This model includes best practices that we share when we train our providers. Even though we are sharing information on this model, it is important to note that you must still follow policy. 


The model may not always align with policy and if there is a discrepancy, you must follow CMS guidelines.   

 

We also had several questions that came up during the trainings that we want to clarify:

  • Crisis plans must still have information and plans for natural disasters and other important potential crises outside of behavioral crises.
  • Meeting minutes must be attached to the POC.
  • Members have to be present, however you are allowed to request permission for a member to not be in attendance.
