First and foremost, I want to share my gratitude for the honor and privilege to serve as your APP Board Chair. Since our very beginning, APP has been a physician-led organization with a governance structure that ensures your voices are heard in all decisions that impact the clinically integrated network (CIN).


Last week, we held our annual APP Board of Directors Retreat, which includes all of the PHO Presidents who shared local perspectives as we set strategy that will build upon our shared success and culture. Here are three things to know and areas we must focus on to drive success in 2024:

 

  • We are truly better together! We achieved significant improvement in 2023 to close gaps in care that matter most to our patients. We closed an incremental 59,000 care gaps over last year and re-evaluated 260,000 more chronic conditions. We approved the network structure changes of the Specialist Incentive Transformation (SIT) to develop appropriate measures and incentives for specialists and we are implementing an interoperable data exchange vendor to ease the burden on the physician for data collection that will comply with the new CMS quality submission standards.
  • We need to focus and make improvements in our Admits/1000 and SNF days/1000. APP has expanded our Care Transitions Program to be payer agnostic for all in-patient and observation discharges at Advocate facilities to support you in completing TCM visits.
  • APP is at an inflection point. Our PCP network is aging, and our board is committed to developing a framework to stabilize and expand our primary care services so that APP can continue to thrive in the future. We must continue to think innovatively and continue to be the disruptor.

Reminder – CT Heart Scans: Self-referrals for CT heart scans are no longer accepted. We will continue to offer heart scans at $49 for early diagnosis of heart disease for patients. Orders should be placed through Epic or as a paper order from an established physician or APC. Self-scheduling will also continue in LiveWell and on our consumer websites. Read More.

Next Epic upgrade April 10: To continuously improve our use of Epic and Epic Connect in providing safe and efficient care for our patients, we conduct periodic upgrades. Watch for more information including education dates and materials as well as additional resources.


Information Blocking Reminders: In alignment with both federal guidelines and Advocate Health Care policy, physicians using Epic, including Epic Connect, must include a reason in the Comments field for any note that is blocked from sharing with the patient in Epic must include a reason in the Comments field. Personal disclaimers within documentation are prohibited. Both disclaimers and blocked notes are regularly monitored. Learn more.

The annual Advocate Health Care required learning, including the mandatory compliance training, will be available in late summer. 

Implicit Bias Training and IL Requirement

Research reveals that implicit biases can create harmful disparities in patient care. This online course will help you better understand implicit and explicit bias and how to recognize, interrupt and mitigate these biases. This training fulfills the Illinois Department of Financial and Professional Regulation (IDFPR) Implicit Bias Continuing Education (CE) requirement. Learn more and sign up.
