W-2 Form Mailings

Every January, W-2 tax forms are mailed out to IHSS providers. W-2s are sent to the MAILING address last reported by the provider and recorded in the State system at the time the W2 is generated. Please ensure your addresses (mailing and residential) are correct to receive your W-2. Addresses can be changed online using the Electronic Services Portal after creating an account or also by contacting the Public Authority and requesting a Change of Address form. If you completed the Live-In Self Certification form for Federal and State Tax Wage Exclusion (SOC 2298) you will not receive a W-2. Reminder: For any tax-related questions or advice, please check with the IRS or a tax preparer.
