One major challenge in the U.S. has been that healthcare funding and incentives have not included resources to address the social causes of health inequities. This has perpetuated the problem. Conversely, several European countries have combined funding for health and social services and created incentives for communities and organizations to address both areas collectively. Over the past 10 years, accountable care organizations (ACOs) have focused on implementing value-based care and new value-based payment models. The impact of this effort is well documented.
Incentivized by the Centers for Medicare & Medicaid Services (CMS) to transform care delivery to improve outcomes, healthcare stakeholders are increasingly turning to value-based care initiatives. Outcomes are now contingent on providers working together across the continuum for shared goals, but hospitals are often not equipped with the resources necessary to do so.
That’s where technology comes into play. To operate more effectively in value-based programs and achieve the Quadruple Aim, hospitals can utilize technology to improve outcomes across five critical, interconnected areas.
As the United States continues to move toward value-based care, a great deal of attention has been given to vulnerable patient populations. Patients with higher needs (and correspondingly higher costs) are among those who would benefit most from healthcare improvement efforts, but those costs can represent significant risk to networks that stake their revenue on cost savings. One such group is the cohort of dual eligible beneficiaries (Duals), those with both Medicare and Medicaid coverage. Health information exchanges (HIEs) are well-situated to help healthcare organizations achieve the triple aim of value-based care—improving population health management strategies, providing better care for individuals, and reducing healthcare costs. Value-based care is a form of reimbursement that ties payments for care delivery to the quality of care provided and rewards providers for efficiency and effectiveness. This form of payment has emerged as an alternative and potential replacement for the fee-for-service reimbursement model, which pays providers for services delivered based on bill charges or annual fee schedules.
