The Department has identified a need for statewide technical assistance to community partners who frequently encounter people in need of post-discharge HCBS, Skilled Nursing Facility/Nursing Facility, or Alternative Care Facility supports, and to the agencies receiving referrals for those people.
The goal of this webinar is to ensure that community partners, Regional Accountability Entity agencies, and Case Management Agencies:
- Have a base-level understanding of options for post-discharge planning
- Are familiar with resources available to teams responsible for safe discharge planning
- Understand roles and responsibilities of safe post-discharge planning for Health First Colorado members
