GaHIN: Making Whole-Person Care Possible

At GaHIN, we know it’s all about the data. Medical and social care providers need greater access to data that can give them a more complete picture of their patients and clients. State agencies need greater access to more complete data about the populations they serve so they can better plan for and deliver services to Georgia citizens, including some of our most vulnerable populations. GaHIN’s new integrated platform not only gives all of these stakeholders access to the data they need but also gives them the ability to securely share the data to help close critical gaps in care.

With GaHIN at the center, clinical and social care providers have one place to go to see a person’s complete longitudinal record and to send referrals to ensure the person receives the proper care – all without requiring any technology other than the Internet.

As more organizations realize what is possible with GaHIN, we are seeing an explosion of interest, with new use cases being defined and new members coming on board every day. This growth equals greater depth and breadth of accessible data:

  • GaHIN’s Central Data Repository is being populated with more data – in terms of volume and different types of data, which now includes the client roster from the Department of Community Affairs' (DCA) with demographic and housing status data for matching across clinical and social data. The Repository already includes a wide variety of clinical data, such as allergies, diagnoses, immunizations, vitals, ADTs, historical lab and radiology results, and social determinants of health.
  • GaHIN’s Community Resource Directory is also growing through direct connections with various organizations, including community-based service organizations like Partners for Home and state agencies like DCA's Housing Resources Directory. We are working to finalize a connection with the United Way’s 211 service directory to populate GaHIN with thousands of Georgia community-based service organizations.

These direct feeds into GaHIN’s Directory make it easy for clinical and social care providers to access the most up-to-date resources available to assist their patients and clients. With GaHIN’s new clinical and social care platform, everyone wins.

We encourage you to contact us and learn what GaHIN can do for your organization as we remain committed to making whole-person care possible across Georgia.

New Training Resources Available

As more member organizations start to leverage GaHIN’s new platform, our operations team has been busy building and making new end-user training resources available. This includes hosting live training and onboarding sessions, delivering a new GeorgiaConnX Reference Guide, and launching a new member support help desk.

These new resources are helping more authorized users get up and running quickly and delivering great value to their organizations and patients. Reach out to us if your organization needs assistance with onboarding or training with the new GaHIN solution.

GaHIN Celebrates Health Literacy

Awareness Month

October is National Health Literacy Month, and here at GaHIN, we understand that health literacy is intrinsically linked to health equity.

Low health literacy often disproportionately affects those already facing barriers to care, including individuals in rural areas, racial and ethnic minorities, and people with lower levels of formal education. Due to difficulty navigating the healthcare system, these groups are more likely to experience poorer health outcomes, higher hospitalization rates, and increased healthcare costs.

As we pursue our mission of making clinical and social data available to those responsible for providing medical care and social services, we are proud to do our part in improving health literacy and health equity for all Georgia citizens.

GaHIN On the Go with Medicaid

GaHIN team members Benita Bowers and Sonya Peacock will represent GaHIN at the annual Medicaid Fall Fair on October 31. They will be speaking with attendees about our new integrated platform that brings clinical and social care data together to help ensure Georgia’s Medicaid beneficiaries get access to the care and services they need.

The event is being hosted by DCH Medicaid division, Gainwell Technologies, and GAMMIS. Stay tuned for more details from the event in our November newsletter.
