Endodontics

A 20-year-old female presents to your practice as an emergency appointment with shooting pain on eating and drinking from the lower left quadrant. Upon oral clinical and radiographic examination, the diagnosis is an irreversible pulpitis with normal apical tissues in the lower left first molar.

How would you manage this deep caries lesion with irreversible pulpitis?

Dr. Thankam Thyvalikakath and her National Dental PBRN study team published the first study in the US to analyze tooth survival after root canal therapy from community dental practices. They gathered de-identified data 46,000 patients from 99 solo and small-group dental practices from in the Network. The data revealed that the median survival time of a tooth after a root canal is 11.1 years and it varied by geographical region as well as type of restoration:

  • Teeth that receive a filling and subsequently a crown last about 20 years.
  • Teeth that receive either a filling or a crown last around 11 years.
  • Teeth that receive no restoration only last about 6.5 years.

“This information can be used to inform dental practice, and help patients and dentists make better care decisions.” said first author Dr. Thyvalikakath, director of the Regenstrief-IU School of Dentistry dental informatics program.

The initial phase of the POPS study was successfully completed, and we are now recruiting additional dental practitioners.

Practitioners from the initial phase have enjoyed this study. A practitioner from the Network's Northeast node, who participated in the study, stated, “I have a patient who had oropharyngeal cancer and couldn't talk as they had their vocal cords removed. They were super excited to use the app and be involved in the study as our communications earlier would be through a family member. They could now text me directly with any of their dental concerns. They could actually put in information about their status post-treatment, and I could respond with a text to discuss what's going on. They’re really excited to be a part of the POPS Study as it has improved their overall dental experience through easy communication.”

If you are interested in pain management after potentially painful dental procedures, you may be eligible to participate.
