REGISTRATION INCREASES DUE TO SELF-MANAGEMENT PROGRAMS NETWORK WEBSITE

The goal of the Consolidated Self-Management Website was to remove geographical barriers to increase access to workshops for all - it has succeeded!


On March 31st, 2022, the Self Management Programs Network was launched to bring together all self-management workshops from the four sub-regions, within Ontario Health West, into one centralized website to maximize program offerings, and improve access to and awareness of self-management programs.


By eliminating the geographical barriers between South West, Waterloo-Wellington, Hamilton-Niagara-Haldimand-Brant and Erie-St. Clair sub-regions, residents can easily browse self-management workshops to determine the best program that suit their timing and needs.


In the first three months of the website from April-June, close to 800 people from the different sub-regions registered for a workshop.


Of the 800 participants, 75% resided in the sub-region, 9% out of the sub-region but within Ontario Health West and 16% came from outside of the Ontario Health West Region.


“This proves that when you eliminate hurdles, residents will take advantage of the offerings and discover other programs to help themselves,” said Sally Boyle, Manager of System Strategy, Planning, Design and Implementation for the South West Self Management Program. “These numbers speak for themselves. We are pleased with the results thus far.”


The website offers people living with a chronic illness, caregivers and providers all the self-management programs available in the Ontario Health West region in one central location. 

WELCOME TO THE MLOHT

We are excited to announce two new additions to our team.


Eric Hendrick is our new Indigenous Health Improvement Facilitator, a shared role with our Elgin OHT partners.  Eric will work with health representatives from the three local First Nation Communities and OHTs to improve health care in those communities.  


Rashmi Gupta is our new Communications and Community Engagement Specialist. Rashmi joins us from the International Federation on Aging and will help us raise awareness of the MLOHT and engage with our partners.


They start September 12th. Welcome to the MLOHT. 

FAREWELL MOHAMED DAHER

Mohamed Daher, our Digital Health Project Manager has decided to pursue education in dentistry in Ireland.  Mohamed has been with the MLOHT since January 2022 and helped us with the Attributed Population Registry and Shared Care Record projects. His final day was August 26. 

 

We are sad to lose such a valuable member of our team, but so happy for him and his exciting new chapter.  Thank you Mohamed for your contributions in helping us to move closer to our vision of a more integrated health care system for everyone.

THANK YOU FOR YOUR FEEDBACK - eReferral via eFax


We have been working with the London Middlesex Primary Care Alliance and Ontario eServices Program to bring you eReferral via eFax.  This project increases the providers' ability to send referrals electronically through Ocean.  


We would like to thank everyone who took the time to complete the short survey about this new functionality. We are excited by the results of the initial phase of the project and are working diligently to move forward in this project.

MIDDLESEX COUNTY TRANSPORTATION NEEDS ASSESSMENT STUDY

Middlesex County is undergoing a Public Transportation Needs Assessment Study to look at transit solutions for county residents, workers, students and visitors.  The goal is to review existing services, identify gaps and develop a plan to address the needs identified.   


Please complete the survey by September 16th to provide the project team with important information to help shape the design of future transit services.

  • Members for a new Privacy Advisory Committee to optimize our ability to share health information with a goal of better connected care.
  • Members for a new Equity, Diversity, and Inclusion (EDI) Collaborative to regularly connect EDI Leads/Groups to efficiently work together on shared priorities.

LET'S CELEBRATE


A pat on the back to our former practicum student Marta Chmielewski who recently won the Master of Public Health Award for the best showcase poster. At the 2022 Practicum Showcase, she presented the work she completed during her placement this year at the MLOHT. The poster titled 'Population Health Management Through a Shared Care Record' highlights the purpose of OHTs and the vision for a better connected health care system. During her time at the MLOHT, Marta conducted interviews to understand end-users adaption to a shared care record.  


"I look forward to using the skills and insights I have gained from my work with the MLOHT to advocate for population health management approaches that make better use of public resources and improve the health of population," Marta noted in her poster.  Congratulations Marta!

  • First Booster Shot Available for Children aged 5 to 11: As of September 1, parents and caregivers can book an appointment for a paediatric COVID-19 booster dose. 


  • ESL Training: The Thames Valley District School Board is providing English as a Second Language training virtually and in person.  Registration for September classes are now open.
