The introduction of the new V28 CMS-HCC Risk Adjustment Model could be beneficial for some ACOs and problematic for others. Although some ACOs may start out being relatively disadvantaged, the outcome is not a foregone conclusion. ACOs have the ability to take advantage of these changes even in 2023 by engaging specialists, prioritizing coding changes by ROI, and implementing year-round HCC gap closure initiatives. ACOs also have the ability to improve long term performance starting in PY 2024 through participant selection, evaluation of early renewal, and track decisions.

In value-based care, it takes a village of professionals to ensure patients are well cared for and practices meet benchmarks. Community health workers (CHWs) are emerging as key players in helping to achieve these goals. CHWs are nonclinical healthcare professionals who support patients’ physical, mental, and social needs by connecting them to community resources and services. In doing so, they make sure patients receive comprehensive care when they are outside of their doctor’s office, while easing the burden on medical offices to do so.

Accountable care organizations (ACOs) are provider contracting models intended to improve population health while controlling cost. With careful design and monitoring they can also serve as a platform for improving health equity. While much of the innovation and research related to ACOs has occurred in the context of Medicare and commercial insurance, Medicaid ACOs may have the most to teach us about how payers can structure their ACO initiatives to improve care for the most disadvantaged patients and thereby improve health equity.

In the ongoing shift from volume to value in US health care, primary care value-based alternative payment models (APMs), such as accountable care organizations, have dominated the landscape when compared to specialty care APMs. Interviews with hundreds of payers and clinicians across various specialties signal that there are ongoing challenges with investing in, designing, and scaling specialty APMs, especially for populations covered by commercial insurers. Several critical gaps complicate specialty APMs’ business case for these stakeholders, including uncertain returns on investment, lack of understanding around optimal outcome and performance measures, and data silos.
