Medical Home Network (MHN) isn’t new to care coordination. For 14 years, the Chicago-based accountable care organization (ACO) has focused on bringing care coordination into the primary care office, using community health workers to provide whole-person care. But now, as a part of the ACO REACH Model, MHN looks forward to a future free from the constraints of fee-for-service and focused on health equity and social determinants of health. |
The U.S. health system has been evolving over the past decade to focus on the quality and value of care. Until recently, though, value-based payment models have not explicitly addressed longstanding racial and ethnic inequities in care. This month, Blue Cross Blue Shield of Massachusetts, the not-for-profit health plan I lead, introduced a value-based payment model that will reward clinicians for closing equity gaps, starting with areas including colorectal cancer screening, blood pressure control, and diabetes care. Four of the largest health systems in Massachusetts have signed these contracts, the first of their kind. We will be sharing our methods and learnings to support other payers across the U.S. that are eager to contribute to a more equitable health system. |

After an intense few years in health care, will 2023 deliver more punches? While 2022 was dubbed a COVID “recovery” year, as patient volume rebounded, health care staffing shortages festered. Burnout prompted physicians to retire, sell practices to corporate owners, or leave traditional health care for other employment. Simply put, 2022 was short on recovery and stability. Even still, 2022 fostered important new trends and discoveries. Despite inflation and recession fears, corporate health care continued its growth undaunted, with new startups and equity-backed practice expansion. |
As the industry moves deeper into value-based payment models and takes accountability for the health of populations, primary care is serving as one of the highest priorities for health systems and networks alike. We are at a critical juncture where independent, affiliated and employed physicians, as well as the employers and health systems working with them are seeking ways to allocate limited resources to provide more coordinated, higher-quality care at a lower cost, and position themselves as the provider of choice in their markets. |
