The Centers for Medicare and Medicaid Services has redesigned its Medicare Direct Contracting Model to an Accountable Care Organization model focused on health equity. The ACO Realizing Equity, Access, and Community Health (REACH) Model will replace the Global and Professional Direct Contracting (GPDC) Model at the end of the year. The new ACO REACH Model will require all participating ACOs to have a robust plan describing how they will meet the needs of people with traditional Medicare in underserved communities and make measurable changes to address health disparities.
On February 24, 2022, the CMS Innovation Center released the details on a new payment model, ACO REACH. ACO REACH represents a series of changes made to Global and Professional Direct Contracting (“Direct Contracting”, GPDC) to respond to criticisms faced by the program. ACO REACH adds in exciting new components aimed at closing health equity gaps in keeping with the Innovation Center’s Ten Year Plan released late in 2021. While ACO REACH and Direct Contracting share most of the same model components, there are some important differences. At a high level, ACO REACH addressed concerns about having funded entities in control of seniors’ healthcare by requiring 75% of the ACO’s governing body be controlled by participating providers...
With the advent of mHealth devices and telehealth platforms, Remote Patient Monitoring (RPM) has gained traction as a legitimate care pathway. Using connected health tools that can track patients at home as they go about their daily routines, care providers can now monitor patients’ vital signs, adjust care plans on the fly, intervene when necessary, and even gather non-clinical data to better understand all the factors influencing the health and wellness of their patients. RPM programs can certainly benefit a hospital, clinic or practice if well planned and run, but they can also be a disappointment if they’re not set up for success. 
In October 1991, the First National People of Color Environmental Leadership Summit brought together close to 300 Black, Latino, American Indian, Pacific Islander, Asian American, and other activists of color to elevate the environmental justice movement, which aims to address the inequitable distribution of environmental risks among vulnerable populations. The movement was founded by people of color with an explicit emphasis on securing “political, economic and cultural liberation that has been denied for over 500 years of colonization and oppression.”
