Please print the sign-up form attached. Complete the form and return it along with your payment.

Payment

Payments are to be made as follows:

• Fill out the request form, VM III Palm Tree Trimming Registration 2023.
• Make check payable to Treetime Design, LLC.

• Mail or deposit your check and completed form in the VM III's mailbox at 7702 E Highland Avenue, Scottsdale, AZ 85251.
