This module covers the lifecycle of a direct referral, how to navigate the Referral Dashboard, and the steps to close out a referral from beginning to end.

Care Team Members will soon receive notifications of housing and health changes, an indication of food insecurity, and missed appointments for CIE participants to provide proactive support.

Provides short-term financial assistance to support housing stabilization including payment of arrears, deposit assistance, utilities, and other approved costs to assist with preventing homelessness for eligible Veteran households. Case management, house navigation, and other supportive services are offered to eligible households that are at imminent risk of losing their housing and have exhausted all other options. Individuals and families will be screened and prioritized as capacity permits. Financial assistance may vary.
Provides housing search and supportive services to homeless, very low-income Veteran families. The Service also assists with accessing VA and public benefits and other community resources.
Active participants may apply for temporary financial assistance that is awarded on a case by case basis, for rent, security deposit, and other types of financial assistance in support of permanent housing.
Provides services to eligible homeless enrolled in SSVF and meets the criteria to receive a shallow subsidy to assist with ongoing housing stability. Case management was provided to ensure housing stability. Recertification for eligibility occurs every two years. The subsidy is determined based on AMI and the fair market rate of the unit.
DescriptionRapid Rehousing for homeless Veterans or who are about to become homeless. Service is for low income 30-50% of AMI, homeless or about to become homeless Hispanic/Latino living in zip codes 92243 and 92231
Provides temporary financial assistance and short-term case management services to assist homeless Veterans and their families with obtaining permanent housing. Case managers assist with locating housing, money management, and referral linkages to a wide range of services. These services include employment opportunities, income benefits, mental health benefits, medical services, and more.

In partnership with the San Diego Foundation, 211 San Diego/CIE would like to invite you to the Coordinated Acess for Financial Assistance (CAFA)listening session.
We understand that coordinating services during COVID-19, specifically, financial assistance, was challenging for many organizations and these challenges may impact access to much-needed resources for community members.
The San Diego Foundation is supporting a pilot program to learn more about ways we can leverage collective coordination and the CIE technology to improve access to financial service resources for community members, community-based organizations and providers.
What: Coordinated Access for Financial Assistance Listening Session

Meeting Goals:
- Highlight the need and opportunity for greater coordination among organizations providing direct financial assistance (i.e.: programs that pay for or provide financial assistance to pay for clients' expenses)
- Discuss the role CIE can play to support coordination efforts
- Share next steps for feedback and insights to support providers and community members

Community Catalyst of California's Veterans Community Services is now Adjoin!
They joined CIE in 2018 and since then provided services to Veteran families who are experiencing homelessness or are at risk of becoming homeless that connect to CIE for services.
Their mission is to create limitless pathways for people to belong where they live, work, learn, and play.
|
|
California Smoker's Helpline is now Kick It California.
They joined CIE in 2018 and since then provided services to individuals that want to quit smoking and connect to CIE for services.
Their program helps Californians kick smoking, vaping, and smokeless tobacco with the help of proven, science-based strategies.
|
|
November is Diabetes Prevention Month
|
Locally, CIE is working to increase awareness with FQHCs and lifestyle change program providers to increase access and education on the importance of identifying if an individual may be prediabetic and what can be done to prevent Diabetes.

Holiday Assistance Programs
|
During the holiday season, 211 San Diego connects the community to holiday assistance programs that help make the holidays possible for individuals and families that would otherwise not be able to celebrate.

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.
Join us for a range of content. Each month we may dive into the latest technology projects and elicit feedback on the platform’s care coordination tools. We may spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.

Join us for the next CIE Ethics (Policy) Workgroup meeting to review some of our current policies, procedures, and steps we are taking to ensure that we are protecting the privacy and security of the clients that we serve. This workgroup is a space where partners can provide recommendations and proposals as it pertains to any policies that involve and affect CIE Network Partners and the individuals we collectively serve.

The Community Information Exchange invites you to join us for an introductory meeting where you will learn about the CIE. After this meeting, you will be asked to join our Community Voice Affinity Group and sign up for a topic of interest.
When: October 22, 2021, at 6:00 PM (PST)
To register for this meeting, you must meet the following requirements.
Experience:
• 18 years or older
• Interested in the way data is shared with health and social services
• Interest in community planning & impact
• Could have experience in seeking social services
Commitment:
• Willing to meet once a month (starts October 2021)
• Willing to meet virtually (*computer and internet needed; can support those without access)
• Willing to provide input or participate in CIE governance
Compensation:
• Participants will receive a $50 compensation per meeting
