Poll: Technology Criteria Prioritization

In partnership with Redwood Community Health Coalition, SC|SU, is working to build out a medical and social system integration platform to support communication, data sharing, closed-loop referrals, and case management between organizations.


You are invited to please complete this Mentimeter poll to rate what user interface and features community members want for the technology. Please participate by or before 1/14/2022.

Upcoming Action Team Meetings

This month, the Sonoma Connect | Sonoma Unidos Action Teams will be working on the agenda items below:


ACE Screening Clinical Implementation Action Team

Tuesday, 1/4 8:30-9:30am

In January, the team will be giving an update on screening metrics and share aggregate data. They will begin discussing workflows between health centers and CBOs.


Neighborhood Resource Centers Action Team

Wednesday, 1/12 3:00-4:30pm

The conversation will focus on resource mapping and assessing gaps including a presentation on the latest survey from NorCal Center for Wellbeing on mapping Community Health Workers in the county.


Community Accountability Action Team

Thursday, 1/20 1:30-3:00pm

Join the team in reviewing the bylaws andof the new advisory board for SC|SU as well as get updates surrounding overarching goals for 2022.


Resource Connection Network Action Team

Monday, 1/31 1:00-2:30pm

Redwood Community Health Coalition is spearheading the request for proposals for the resource and referral platform. Participate in this months meeting to provide feedback on that process.

Learning Opportunities

Community Health Worker/Promotorx Core Competency Training


The Center for Wellbeing is offering a FREE online training for Community Health Workers/Promotorxs. There will be weekly sessions on Fridays, starting January 14, 2022 from 2-5pm. Please register by January 10 to participate.
