Patient Advocacy: The Cycle of Paying it Forward

It was the spring of 2020. Flip the Pharmacy Change Package for December 2021 Hypertension Progression

The change package is an implementation guide which is intended to help pharmacies to improve their workflows, increase meaningful patient care experiences, and understand documenting eCare Plans.   During December, the focus is follow-up and monitoring of patients, especially those with hypertension. Prepare to check blood pressures during January by a non-pharmacist support staff person. A training is listed within the change package to help pharmacies prepare.   Continue to enroll patients into medication synchronization as this is very important in order to take on new practice transformation initiatives.
