Message from our Director, Adriana Arrizon:

Today I sat down to enjoy dinner with my sons in awe of how much they have grown. Then I remember they are not the only ones that have grown. I have grown. You and I have grown together. And my heart is full, and I feel deep gratitude for you, for my boys and the events we faced together the past year. And as I welcome the new year in celebration of our work and growth together, I can’t help feeling a need to take pause and reflect on the past year before I move forward.


My body, mind, and spirit are telling me to pause, listen, and take time to create space for silence and reflection. Two thousand twenty-two was not a silent year. We continued to experience the effects of social and environmental impacts like war, economic uncertainty, and severe climate changes in our body, mind, spirit, and ultimately our collective work. And before we courageously face new opportunities and challenges together this new year, I encourage pause and reflection on any needs or impacts your body experienced.


Through reflection comes the acknowledgement that change, and growth are not without pain or impact. And only after we acknowledge pain or impact can we begin our pathway to healing, creativity, ingenuity, and the energy needed to change the things we need to change. As we step into this new year, I invite you to take time to reflect, rejuvenate, and celebrate yourself because, this is the work.

In 2023, SC|SU will continue to intently pause, listen, and reflect on our collective work with by centering our co-created values:

  • Be human centered 
  • Dismantle systems of oppression 
  • Ensure culturally responsive system and programming 
  • Eliminate ACES and SDOH 
  • Share efforts and resources 
  • Collect data guided by anti-racist and indigenous practices 

 

Don’t hesitate to share with us your reflections and intentions for our collective work this year!

Community Advisory Group Opportunity - $1,000 Honorarium

Sonoma Connect | Sonoma Unidos is a Participating Network Organization (PNO) in a research project called "Research to Pursue and Advance Racial Equity (RePARE)" that is being facilitated by Health Leads with eight community networks across the U.S. 

 

RePARE is a research project to discover how Networks of Care (such as Sonoma Connect | Sonoma Unidos) across the nation impact the community and how to design and build them. Networks of Care are systems that make it easier to connect people with available resources in health and social services and may leverage technology such as the Sonoma County Resource Connection Network. 

 

We are seeking a community member, with experience trying to access community resources and social services for yourself and others, to participate in the RePARE Community Advisory Group. Each Community Advisory Group member will get a $1,000 honorarium.

This month, the Sonoma Connect | Sonoma Unidos Action Teams will be working on the agenda items below:


ACEs Clinical Action Team 

Thursday, January 5, 2023 from 2:00-3:00pm

This meeting will review work accomplished in 2022 and give participants an opportunity to help guide the direction of this action team in the New Year. 



Community Accountability Action Team 

Wednesday, January 11, 2023 2:30pm-5:00pm


We are excited that our Diversity, Equity, and Belonging Workshop will begin this month!


Dates: January 11, January 25, and February 8, and February 22.

Time: 2:30pm-5:00pm **Updated Times**


Our DEB Workshop will begin on January 11, 2023 and will continue every two weeks on January 25, February 8, and February 22. Thank you to everyone who expressed interest in participating in this training. We look forward to seeing you soon!


Resource Connection Network Action Team  

January 30, 2023 from 1:00pm-3:00pm

This meeting will provide important updates on the pilot launch of the Resource Connection Network, as well as information on next steps for training and deployment of the platform. 

CalAIM Corner

Peer Network Meeting in January

SC|SU partners are invited to join the next CalAIM/Enhanced Care Management Peer Network meeting, led by Aliados Health (previously Redwood Community Health Coalition), on Wednesday, January 18th at 11am to learn how community-based organizations can coordinate and help improve care and health outcomes in our community through CalAIM participation.

Collaborative Planning and Implementation (CPI)

The Population Health Innovation Lab (PHIL), is supporting the Collaborative Planning and Implementation (CPI) initiative to support regional collaborative planning efforts to implement ECM and Community Supports (including PATH, Incentive Payment Program, and CalAIM funding initiatives). The Population Health Innovation Lab will hold two listening sessions.

Community Partner Events 

Sonoma County Office of Education Event

The Sonoma County Office of Education is hosting an event on January 31, 2023 from 8:00am-4:00pm (please see flyer and share)! Participants will collectively reimagine a behavioral health approach that centers justice, hope, and healing for our most resilient youth. SCOE wants to create a day in which those with both lived and professional experiences tell stories, offer and experience hope, celebrate strengths, call-out injustices, and commit/re-commit to our togetherness on this journey.

Hanna Institute Training: Supporting LGBTQ+ Youth

Date: Thursday, January 19, 2023

Time: 9:30 AM - 12:30 PM PST



Penelope French from the Trevor Project joins Hanna Institute to create dialogue around being an adult ally for LGBTQ youth by providing a basic framework of understanding lesbian, gay, bisexual, transgender, queer & questioning (LGBTQ) youth and the unique challenges they often face.


Supporting LGBTQ+ Youth will take a deeper look into the Trevor Project’s CARE (Connect, Accept, Respond, Empower) concept which will provide attendees with an interactive and intensive overview of suicide among lesbian, gay, bisexual, transgender, queer & questioning (LGBTQ) youth, and the different environmental stressors that contribute to their heightened risk for suicide.
