THE SHORT REPORT

Hi everyone,

This past weekend, we observed Remembrance Day and I find myself reflecting on the tremendous amount of conflict we are experiencing in our world today. I know it is affecting all of us; and for many of our Department members it is impacting them very close to home. This, on top of the daily stresses we are experiencing in our medical system, can be difficult to carry alone. It continues to be a really important time to lean on each other and carry these burdens together. A great place to start is with small acts of kindness. I challenge you to be intentional about engaging in at least one act of kindness each day as a way to promote peace on our teams and in our communities and our world. The ripple effect of kindness cannot be underestimated and it’s positive impact on our wellbeing is tremendous. (Kindness matters guide | Mental Health Foundation | mentalhealth.org.uk)

The Department continues to work hard with NSH to improve access to acute care services for the patients we serve and to optimize the work environment for our staff and learners. The QEII has recently opened a new assessment zone which is seeing an average of 21 patients per day shift outside of the traditional Emergency Department (ED) space. The QEII has opened an overflow space on the third floor of the Halifax Infirmary which can hold up to 16 patients. This space is being used for more stable patients, including our Department of Medicine Emergency (DME) patients when appropriate and patients close to discharge from hospitalist and medicine services. So far this seems to be having a positive impact on patient flow and the work environment in the ED. We continue to work with our Hospitalist and Emergency Medicine groups toward longer-term solutions to improve our patient pressures in the ED and we hope to see these coming online early in the new year. I so appreciate all the work you do every day to take care of the patients we serve and to train our physicians of the future.

Below you will read about our welcomes and farewells to Department members. I want to mark one farewell in particular. Connie Tone recently left our world all too soon. Connie joined our department in 2009 in our Physician Services office and was a loyal employee and department member for 12 years until her retirement in 2021. Connie’s life was sadly cut short after a courageous battle with cancer. Connie always went above and beyond at work and at home and I want to recognize the tremendous value she was to our Department. The Department has donated to Chalice in Connie’s memory.

Lastly, I want to thank Dr. Chris Gray for his leadership as our Director of Education. Chris is leaving this role after two terms and helping us navigate two strategic plans. The Education Committee will now fall under Dr. Shelly McNeil as Deputy Head Academics and we look forward to what she will bring to this role.

I hope you enjoy the content of this month’s newsletter.

Don’t forget to send us your stories and ideas.

Stay well,

Christine

CONFLICT AND CULTURE

To help further the work of the strategic plan, we are focusing on strengthening our toolkit to improve culture and conflict in our workplace. Content submitted by Diane LeBlanc, Basia Solarz and Brennan Dempsey.

Managing Fight or Flight Responses

Working in healthcare is undeniably stressful, often triggering intense emotional reactions.

This is often the case with leadership in the healthcare system as well. While it's important to regulate our emotions, our instinctive fight or flight responses - evolved to handle threats - can sometimes take over. In the past, this response helped our ancestors survive by fleeing or hiding from predators. In the present, it kicks in when we sense workplace threats and our bodies flood with stress hormones making us hyper-alert. Continuous exposure to these stressors can lead to health issues like hypertension, which makes it crucial for leaders to manage these responses effectively.

Mind

There are evidence-based methods to reduce this risk and help people control their emotions in high-pressure situations. Improving emotion regulation not only promotes personal well-being but also protects our ability to think rationally, leading to better decision-making when responding to stressful situations. For instance, practicing mindful observation enhances our awareness of non-verbal cues, such as facial expressions. Regular mindfulness sharpens our ability to stay present, allowing us to discern even subtle emotional signs. This enables us to actively listen, ask relevant questions, and express empathy, which can reduce fight or flight reactions.

Group of people

Fostering a supportive team environment is equally vital. Open discussions about challenges and shared experiences can alleviate emotional burdens. Peer support and mentorship programs create avenues for physicians to learn from each other, enhancing their ability to navigate complex situations. Additionally, integrating training programs focused on emotional intelligence and non-verbal communication skills equips healthcare professionals to interpret cues and situations accurately, even amidst stress-induced fight or flight responses.
