Updated Materials
New versions of the Uniform Application (Version 2.3), Clinic Client Excel Application (Version 12.1), and Federal Poverty Guidelines have been posted to the CICP Provider Information web site. All CICP Hospital providers must begin using Version 2.3 of the Uniform Application and all CICP Clinic providers must begin using Version 12.1 of the Clinic Client Excel Application on Tuesday, April 1.
We've changed the CICP email address to hcpf_CICPCorrespondence@state.co.us. Please update your email address lists to use the new address and remove the old one.
