All-Pro Parent Breakfast

All fifth-grade parents and students are invited Friday, September, 29 to an All-Pro Parent breakfast in the IMS cafeteria from 7:15 - 7:40 a.m. All-Pro breakfast comes with a message for both parent and child to take away. This is a FREE event for all.

TrueCare is BACK!!

We have a new number to text for attendance-related events. Please remove the old number and add this one to your contacts: (484) 255-8384. New families or existing families with new students can register for the program here: True Care (LINK)

As always, please report absences, late arrivals, or early pick-ups to the office by text (484-255-8384), email (skennedy@independence.k12.oh.us), or phone call (216-642-5865).

For ALL appointments, bring in a "School Excuse Note" from your provider to have the absence "Medically Excused."

Last but not least, for upcoming non-medical absences of 2+ days, please complete and return the "Planned Absence Form." This form gives the school advance notice of your absence and teachers an opportunity to collect make-up work for students and can be found here on the ILS website: ILS (LINK)

GRADE LEVEL LEARNING GUIDE REFERENCES

Parents and Students,

Attached you will find Medication Forms for your child’s 2023 8th grade class trip. The attached medication forms comply with the Independence Board of Education, as well as the State Laws, on the distribution of medication to students during school trips. There are 3 forms attached: Field Trip Over the Counter Medication, Non-Prescription Medications, and Prescription Medication. Students MUST turn in the Field Trip Over the Counter Medication form(LINK). This form indicates whether you wish a chaperone to dispense medication to your child while on the trip, if needed. The other 2 forms only need to be completed and signed if your child needs to take additional medication not listed on the Field Trip Over the Counter Medication form. Please note that without a doctor’s signature on the Prescription Medication form, we will not be able to give your child prescription medication while on the trip. If you do not see the medication listed on the Over-the-counter Medication form, then the additional Form Non-Prescription Medication will need to be filled out and turned in before the trip.  Medication Forms are due FRIDAY, September 22.

If you have any additional questions please contact me via email or school phone.

You may also contact our district nurse, Amanda Adler, with any questions via email at aadler@independence.k12.oh.us

**Please note that a hard copy of these forms was passed out to the students today and should be coming home with them.

Thank you,

Mrs. Josephine Vecchio
