Cohort Celebration!

Thank You From DHS and ICI

The Minnesota Department of Human Services and the Institute on Community Integration are pleased to recognize our local organizations and counties who have put in time, resources, and continuous efforts to support those they serve using positive supports and person-centered practices.


The past few years have changed the landscape of the way we provide for and serve those in our communities. As a team, DHS and ICI created a state-wide recognition process for organizations or counties involved with organization- wide positive supports to:


  • Acknowledge cohort organizations' and counties' efforts toward systems of positive support;
  • Incentivize cohort organizations and counties to participate with the Tiered Onsite Evaluation Tool (TOET), critical for monitoring implementation and action planning.


Three levels of recognition were identified for the inaugural recognition process. These levels were defined and used to determine how to best recognize organizations or counties that have been engaged with the Positive Support Cohort Model of the past few years. The levels are summarized below.


Early Adoption and Installation

This level of recognition includes organizations and counties new to this process, or those who have been involved but continue to define what organization-wide positive supports look like with the services they provide. 


Criteria for this level: This level of recognition involves reviewing an organization or county's initial implementation of organization-wide positive supports, or implementation over time. Evidence of ongoing implementation includes: attending Cohort related training events, attending regional team meetings, having shared PC coach meeting minutes or PC/PBS team meeting minutes with ICI, and/or engaging in the TOET at some point since the initial team training. 


Recognition at this level includes: The organization or county name listed in a special issue of the Positive Supports Newsletter, acknowledgment video from MN DHS, and the organization or county name listed on mnpsp.org. 


Initial Implementation

This level of recognition includes organizations and counties who have been involved with the Cohort process, show evidence of implementation, and regularly monitor their efforts.  


Criteria for this level: This level of recognition involves reviewing an organization or counties' implementation of organization-wide positive supports over time AND their completion of 80% or more of the TOET annual review opportunities since their initial training.   


Recognition at this level includes: The organization or county name listed in a special issue of the Positive Supports Newsletter, acknowledgment video from MN DHS, and the organization or county name listed on mnpsp.org, a personalized thank you letter from MN DHS, and a range of thank you items (e.g., mugs, pens, notepads). 


Legacy

This type of recognition is intended to celebrate organizations and counties that have made significant contributions to the work of Positive Supports in Minnesota. Although the type of recognition varies, the Legacy award highlights contributions of new work to the State on different activities related to organization-wide implementation of positive supports, and involvement that has helped move the cohort system forward.  


Recognition at this level includes: The organization or county name listed in a special issue of the Positive Supports Newsletter, acknowledgment video from MN DHS, and the organization or county name listed on mnpsp.org.

Future Levels of Recognition

Additional levels of recognition will be made available for future years of recognition! Details for these levels will be provided as they are launched. 


Evidence of Implementation with Outcome Data

This level of recognition involves meeting the criteria for Initial Implementation PLUS being able to provide evidence of high-fidelity implementation and impact on valued outcomes.  


Advanced Implementation Across Three Tiers of Support

This level of recognition involves meeting the criteria for Evidence of Implementation with Outcome Data PLUS evidence of scaling organization-wide positive supports systematically across all three tiers of implementation.
