Update: Implementation of Health Equity Resource Community Program
Following the August 11 meeting of the CHRC and Health Equity Resource Community (HERC) Advisory Committee, the Advisory Committee launched a public comment period and received comments from an array of stakeholders. These comments will guide the development of the Pathways to Health Equity Call for Proposals and can be accessed here.
The Pathways Call for Proposals will be presented to CHRC Commissions for discussion at the next public meeting of the CHRC on Wednesday, October 6 (virtual). Information to participate in this meeting virtually can be found here. The CHRC anticipates that the Pathways Call for Proposals will be issued shortly after CHRC meeting on October 6.
After the Pathways Call for Proposals is issued, the CHRC and HERC Advisory Committee will hold a series of public outreach events to inform communities and potential applicants about this new funding opportunity. A statewide virtual launch meeting will be held on Thursday, October 14 from 4:00-5:30 pm. In addition, the CHRC will host a virtual “Frequently Asked Questions (FAQ)” call for potential applicants on October 19 from 10:00-12:00 pm. The regional forums will be held both in-person and online. More information about the FAQ call and the regional forums will be posted on the HERC webpage.

Chesapeake Health Care utilizing CHRC grant to launch School-Based Health Center in Somerset County
Chesapeake Health Care, a Federally Qualified Health Center, is implementing a two-year grant from the CHRC to open a school-based health center (SBHC) at Washington High School in Princess Anne, Maryland. The SBHC will provide an array of health services for students throughout Somerset County, on-site and through telehealth. A press release about the program can be found here. Michele McIntosh, CRNP (pictured), will be staffing the SBHC.
Since 2005, the CHRC has awarded 25 grants totaling over $5.8 million to support SBHCs and school health programs in 14 jurisdictions. Click here for more information about CHRC grants supporting SBHCs and school health.

Jewish Community Services expands program to serve vulnerable residents in Baltimore
Jewish Community Services received a grant from the CHRC in 2021, to support the expansion of its Patient Care Connection program in the greater Baltimore area. The program partners social workers with local primary care physicians’ practices to address the unmet needs of lower income patients who are elderly or have disabilities. Patients and caregivers referred to the program receive a comprehensive human services assessment and are connected to community services and resources such as home health care, meal delivery, transportation, medical and prescription insurance, caregiver education and support, emergency response and medication management devices, and more. Gaston & Porter hosts youth vaccine webinar
The Gaston and Porter Health Improvement Center, Inc., a CHRC grantee, will host a free Zoom webinar, “Let’s talk about COVID-19 and the Delta Variant and Vaccines with Youth in the Black Community,” on Thursday, October 28 at 7:00 pm. Featured speaker Dr. Qiana Matthews, Professor of Microbiology, Alabama State University and her youth team will discuss why young people need the COVID-19 vaccine and how to prevent infection and stay safe. Horizon Foundation issues health equity grant opportunity
The Horizon Foundation is requesting proposals for its 2021 Racial Equity Collaborative Grant. Grantees must be based in Howard County and will work collaboratively on a joint equity-related policy. Interested applicants must attend a virtual community listening session on Saturday, October 23. Applications will be due on November 7. Click here for more information.

Rural Healthy People 2030 Survey
The Southwest Rural Health Research Center at Texas A&M University, with funding from the Federal Office of Rural Health Policy, invites individuals involved in rural health to participate in a survey as part of the Rural Healthy People 2030 project. The survey should take approximately 20 minutes. The deadline for participation is October 31, 2021. A link to the survey can be found here.
