The Mount Sinai Health Partners Board of Managers provides members the opportunity to steer our network’s goals, strategies and initiatives in population health management. The Board of Managers sets MSHP policies, approves procedures and provides strategic direction for our network’s Clinical Integration Program. A total of 5 seats are up for nomination to the Board of Managers representing both voluntary physicians and physicians employed by Mount Sinai Health System:

Next Condition Management CME Event: Tomorrow, September 21st
Join us tomorrow, September 21, from 6:00-7:30 pm for the MSHP Condition Management CME Event on managing COPD and Peripheral Artery Disease. Dr. David Steiger, Dr. Peter Faries, Dr. Jeffrey Olin, and Dr. Soma Brahmanandam will share the latest evidence-based practices for managing these chronic conditions in the outpatient setting, including:
- Optimal screening, medical management and criteria for medical/surgical referral for Peripheral Artery Disease (PAD).
- Classification, management and treatment excellence for patients with COPD.
- Key evaluation steps and optimal collaboration, with multidisciplinary team members and specialists for management for PAD and COPD.

Upcoming Payments
Community-based primary care physicians received Q1 2021 care coordination payments in late August. Later this month, we will be issuing Q2 2021 care coordination payments.

Review Your Customized Reports
A new suite of customized CI performance and patient opportunity reports were placed in your practice’s OneDrive account on September 13th. Please be sure to view these reports to see how you’re performing on quality, clinical documentation and utilization. The patient opportunity report includes patient-level information on care and documentation gaps. Use this report to inform your patient outreach strategy.

Complete Your Privacy Training and Practice Communication Requirements This Month
All providers must complete the 2021 HIPAA compliance training module in the Clinical Integration Learning Center (online via the Provider Portal) and the Practice Communication Requirement (verifying your online practice profile) by the end of this month. For details, please refer to the communication you received from us on August 2nd regarding your CI Program status and outstanding participation requirements and deadlines. Please do not delay in completing them.

MOC Training for SNP Providers

CMS mandates all providers participating in the VIP Bold and Reserve Networks to complete the Special Needs Plan Model of Care (SNP MOC) training annually.
