What is a Medical Home?
What is a medical home? 

"The Patient Centered Medical Home (PCMH) is a partnership between the patient, Primary Care Provider (PCP), and a team of healthcare professionals to provide coordinated services that focus on the patient's total health needs." -Department of Health and Welfare PCMH Brochure

It is an approach to provide comprehensive primary care to adults, youth, and children. The PCMH will broaden access to primary care, while enhancing coordination.

Only three seats available!

Idaho Parents Unlimited’s Work of Art Program is for students ages 14-21. This virtual session begins on July 19th, 2022 at 7 PM MTN.

You must be registered through your VR Counselor. If you do not know who your counselor is or how to contact them, contact us and we will connect you to your VR Counselor.

Contact us no later than Tuesday, July 12th, 2022. Once all seats are filled registration will be closed.
EveryLife Foundation's Rare Artist Call for Submissions
The EveryLife Foundation for Rare Diseases is holding its Rare Artist Competition. Submissions are open until July 13, 2022 Details found here:
