Emergency Food Bag Operation
"DayBreak Adult Care Center's Food Program was established in early April of 2020 after the Coronavirus Pandemic spread through Alameda County rapidly in Mid March. DayBreak's Food Program answered a need to assist seniors with getting nutritious foods while staying at home to avoid becoming victims of the deadly virus. The Food Program began distributing weekly food bags to qualifying senior participants age 60 and older who met specific qualifications. Our Program accepted referrals from various entities and today, the Food Program services over 120+ senior participants on a weekly to bi-weekly basis.
Our Food Program Cares: DayBreak's Food Program deliverers conduct welfare check in's with our senior participants when delivering their weekly/bi-weekly food bags to their doorsteps. If there are additional nutrition needs that we can accommodate per request, we support them however we can. Our Food Program also addresses other needs that may come up for our participating seniors. We also make referrals to other programs like Adult Protective Services (APS), Geriatric Assessment & Response Team (GART), as well as provide other supports within our agency like social calls through Caring for the Caregiver, medication pick up support through DayBreak's Rapid Response Case Management Program, and fall prevention solutions through DayBreak's Senior Injury Prevention and Medication Safety Program. During this time of crisis, DayBreak Adult Care Centers is a lifeline for some participants we serve who have limited supports."
