Requesting Vaccination Cards

According to CDC guidelines, students that are fully vaccinated are not required to quarantine if they are considered a close contact with a positive COVID case. The main goal of this school year is to maintain in-person learning as much as possible. If your child has been vaccinated, please submit proof of vaccination to your student’s school nurse to avoid future quarantines.

If you choose to submit a copy of your child’s vaccination card, this information will be treated in the same manner as all medical information: with the utmost confidentiality, inline with FERPA and HIPAA regulations. This information will not be released to the public in any way and will simply be used to make informed decisions for contact tracing purposes.

Thank you for your assistance and if you have any questions, please reach out to your school nurse
Keeping Our Community Healthy & Well

Keeping our NASD community healthy and well is a shared responsibility between our families and staff. To help the NASD community safely continue full in-person instruction, activities, and athletics, the District is offering two voluntary opportunities for our families to receive a COVID-19 vaccination or a flu shot.

NASD COVID-19 Vaccination Clinic on Sept. 16
  • Action Required by Sunday, September 12
  • Full details can be found on the NA website.
  • Parents/guardians of eligible students (12 and older) and staff members must register for the vaccination through this formThe form must be completed no later than noon on Sunday, September 12.

NASD Flu Shot Clinics - October 2 & 6
School Bus Seat Assignments

For contact tracing purposes, the District has assigned students a seat on the bus. The seat assignment translates to a seating chart that is used to identify close contacts on district transportation for quarantine purposes. It is important for students, staff, and families to work together toward students sitting in their assigned seats to avoid unnecessary quarantine.
