Historically, our health systems and social service delivery infrastructure have not been responsive to or inclusive of the true needs of communities and vulnerable populations, especially people of color. Community Information Exchanges have the potential to promote equity and inclusivity through their community-led model. Drawing from current events, including the COVID-19 pandemic, CIE Summit 2021 sessions will thoughtfully analyze and discuss how to move CIE planning and implementation forward using community-led strategies that enable inclusive and equitable community input and participation.

INTERFAITH COMMUNITY SERVICES
Provides social services such as emergency food, hygiene packets, day bus passes or gas vouchers for documented medical or job needs, partial financial assistance for applicable prescriptions, assistance to pay for driver's license, ID, and birth certificates (some limits and criteria do apply, provided based on availability), utility assistance, access to benefits, crisis intervention, immigration, and legal aid services referrals, diapers and formula (when available), and additional community referrals as needed.

Rental assistance is available for residents within the City of Carlsbad (some limits and criteria do apply, provided based on availability). Case management support is available for employment, housing stabilization, and self-sufficiency.

Administers the City of Carlsbad CDBG grant, which assists very low-income households and households in severe emergency situations. Those seeking rental assistance must be or plan to be City of Carlsbad residents. Landlords must complete a W9 and accept payment from a 3rd party provider. Documentation is required. Some limits and criteria do apply, provided based on availability.

Case management support is available for employment, housing stabilization, and self-sufficiency.

Case management support for housing stabilization, employment, and self-sufficiency for individuals experiencing, or at risk of, homelessness. Individuals will gain initial access to basic needs and other resources that may be available to address their housing crisis.

Access point staff may assist with shelter applications, complete eligibility screenings for various resources, make emergency service referrals, and have conversations around housing stability options. It is important that clients remain in contact with the staff at access points and other homeless dedicated providers to ensure their needs and goals are appropriately identified and addressed.

A special Superior Court session for defendants experiencing homelessness or at risk to resolve misdemeanor offenses, minor infractions, traffic issues, and outstanding warrants that occurred in San Diego County. Clients will work closely with an advocate(s) in case management to receive services and resources.

Fines are cleared based on achievements and accomplishments that lead to self-sufficiency.

Case management program is about 3-4 months long.
Will need to attend court session(s).

Housing Assessment

This tool includes a series of questions to help guide a conversation about someone’s housing status and needs to help determine the most appropriate supports in our community.

Nutrition Assessment

Referral Management Dashboard Upgrade
  • With the new referral management dashboard, you will be able to make bulk updates to direct referrals.

Case Management Dashboard  
  • The feature is customizable, so you can quickly review your client base faster than ever before. With one click, view a list of the clients your organization that consented into CIE or a list of your recently viewed clients. 

Alerts  
  • Care Team Members will soon receive notifications of housing and health changes, an indication of food insecurity, and missed appointments for CIE participants to provide proactive support.

To access the assessment and other tools available in CIE, you are required to complete CIE Care Coordination E-learning and complete a short quiz at the end. To access the training, please contact our CIEhelpdesk.

In August, we are pausing the CIE Utilization reports and other CIE activities as we prepare for the 2021 Virtual CIE Summit from September 8-10. We will resume CIE utilization reports and our normal operation on September 13, 2021.

CIE MONTHLY TRAINING OPPORTUNITIES
Learn how to navigate the CIE platform, search for clients, send direct referrals to other partners, add program enrollments and care teams, and more.

During the month of September, there will be two opportunities to join a CIE Training on September 15, 2021, and September 22, 2021.

CIE PARTNER GATHERING

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.

Join us for a range of content. Each month we may dive into the latest technology projects and elicit feedback on the platform’s care coordination tools. We may spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.
