2019 NCAFCC Annual Outcomes Survey Update
Completion of the 2019 Annual Outcomes Survey was due on
February 29,
2020. All surveys will go through a review and data cleaning process. Any findings in question will be forwarded to respondents for clarifications. If your organization receives an email correspondence regarding survey review findings, please respond as soon as possible.
All NCAFCC members were asked to complete Sections A – C of the survey, but to be eligible for the Outcomes Grant funding opportunity, members must have also completed section D.
Patient-Centered Medical Home (PCMH) Transformation Initiative Announcement
We are glad to announce that the PCMH Transformation Initiative will be extended in 2020. Up to 12 clinics that participated in the initiative in 2019 will be offered the opportunity to participate in 2020.
Roadmap to Health Equity Update
For those of you that may not be familiar, Roadmap to Health Equity is an initiative led by Americares, Loyola University, the National Association of Free and Charitable Clinics, and free and charitable clinic stakeholders across the country. NCAFCC staff and many NCAFCC members have been serving as a part of this initiative. This national effort will help demonstrate and evaluate quality of care and lay the foundation for addressing health inequities impacting our most vulnerable patient populations.
The objective of the initiative is to design an infrastructure to enable clinics across the country to collect and report on a standardized set of quality measures and sociodemographic factors. Thus, laying a foundation for improvements in care and giving clinics a tool to demonstrate quality of care and to assess their progress in promoting health equity.
2019 Roadmap Committee’s Highlights included:

Draft introduction for the Quality Measure Starter Set User Guide,

Selecting Vision Tree (software vendor) to build the Roadmap to Health Equity
