Dear Valued IMPACT 211 Resource Provider,

What if there was an intuitive tool that helped you find the right referral for your client/patient at the right time, make an easy connection, and track whether or not the individual followed through? Now...there is.

We’re excited to introduce you to IMPACT Connect, a collaborative partnership of agencies working together to make social services more accessible and navigable to the people who need them. Mirroring other successful community information exchanges across the country, IMPACT Connect allows you to efficiently collaborate with other providers across all sectors using a shared language and shared outcomes to seamlessly make and close the loop on referrals. Together, we can connect people to the resources they need early and effectively, track outcomes to improve client flow, and identify and address gaps in the system.
Meet our IMPACT Connect Partners
We're thrilled to have Advocate Aurora Health, Children’s Wisconsin, Froedtert & the Medical College of Wisconsin health network and Sixteenth Street Community Health Centers as IMPACT Connect foundational partners. We're also actively recruiting partners in housing, child welfare, mental health, corrections, and other sectors to create a true community-wide system. NowPow, the innovation leader in digital health products for complex populations, provides the technology platform that includes a simple electronic screening tool, automated resource identification, and the ability to close the information loop on referrals.
