D O C T O R ' S -- C O R N E R

Concerning the COVID-19 epidemic, our major concern is about testing.

It has been a hassle to be able to test as we need and would like to. There are limited numbers of testing sites, with the majority at Urgent Cares. I’m writing you about new Testing Centers opened in the area to test for acute COVID-19 symptoms by the Maryland Department of Health. These centers are required to have a medical order from the physician who ordered it based on specific criteria. Please check the link below:

The symptoms of Covid-19 have been wide and those that are eligible for testing are those with fevers, coughing, shortness of breath, chills or repeated shaking with chills, muscle pain, headache, sore throat, new loss of taste or smell, and in more severe cases, pneumonia (infection in the lungs). The criteria for who has priority for testing are based on CDC recommendations:

PRIORITIES FOR COVID-19 TESTING (Nucleic Acid or Antigen)

High Priority

  • Hospitalized patients with symptoms
  • Healthcare facility workers, workers in congregated living settings, and first responders with symptoms
  • Residents in long-term care facilities or other congregated living settings, including prisons and shelters, with symptoms
  • Persons identified through public health cluster and selected contact investigations

Priority

  • Persons with symptoms of potential COVID-19 infection, including: fever, cough, shortness of breath, chills, muscle pain, new loss of taste or smell, vomiting or diarrhea and/or sore throat
  • Persons without symptoms who are prioritized by health departments or clinicians, for any reason, including but not limited to: public health monitoring, sentinel surveillance, or screening of other asymptomatic individuals according to state and local plans.

We refer children to Children’s Hospital in D.C. at the Trinity University location, during which, two days per week, weather permitting, they perform drive-through testing.

We test for COVID-19 at the office but we are limited by how many kits Labcorp sends us. We are waiting for point-of-care (in-office) testing, such as with the Abbott test, which only takes 15 minutes to provide a result but, so far, it is not available for small practices. We are on the waiting list for the test.

There are currently developments in testing for COVID-19 antibodies, which demonstrate a previous COVID-19 infection and whether the person has recovered from the disease, and how widespread the disease is in the community. There are many companies working on developing these antibodies tests. The science of antibody testing is evolving and changing fast. It is not clear yet whether or not the presence of antibodies in the body means permanent immunity with complete protection.

It is very important to know if available antibodies tests are able to differentiate well between antibodies for the common annual coronavirus and antibodies for the specific COVID-19 virus strain. There is a new test approved by FDA seems very promising on being specific for the COVID-19 antibody done with just a finger-stick.

The Labcorp home testing, that has been announced on the news, currently is only implemented for first responders and healthcare personnel working in the hospitals. Quest also has an antibody testing to purchase on-line.

As you read this, the situation for testing is getting better, but there is a lot of room for improving and learning.

The Importance of Keeping a Routine

It is real that the COVID-19 has changed our lives and the lives of our kids. Our routine has changed because we all have to make the effort to stop the spread of this disease. The most important change that we have to practice is to maintain social distance, which has altered the way we socialize, work, and learn. Keep a positive attitude and be grateful what you have.
Many professional organizations such as the American Academy of Pediatrics, the American Psychological Association among others recommend the following ideas to reduce the family stress and chaos caused by the lack of routines:

Talk with your kids about their daily routines, ask them about their ideas, and program the schedule together. Put the schedule on a visible place for all.
  • Identify a place without distractions where your kids can do homework.
  • Have clear times for learning, exercise and breaks. For example, 20 minutes of class assignments followed by 10 minutes of physical activity or just relaxing, but without engaging in any activity that could make difficult to go back to the previous activity (ex: videogames or TV).
  • If you are also working from home, try to have the breaks together.
  • Try to have structured times as a family to include time for meals, snacks, and enjoy family time in the evenings, playing, reading, watching a movie or exercising together.
  • Maintain before quarantine bedtime routines as much as possible during the week to make sure everyone gets enough sleep.

https://coronavirus.maryland.gov/pages/symptoms-testing

https://www.cdc.gov/coronavirus/2019-nCoV/hcp/clinical-criteria.html

PRIORIDADES PARA LA PRUEBA COVID-19 (ácido nucleico o antígeno)

Alta prioridad
