SPOTLIGHT: Site Visits in Jackson, TN  memphis

Last month, Account Managers DeBorah Ashton, Logan White, and Mary Olson had the opportunity to visit with two of our network partners in Jackson Tennessee, Pathways Behavioral Health and Faith Health Center. Both sites are strong advocates for their respective communities and showed the team the importance of providing health education along with medication to the patients in need.

Pathways Behavioral Health Services has been a part of the Dispensary of Hope Network since early 2019. Located next to a hospital, this partner provides mental health medication as well as primary care medications. This location has partnerships with both the hospital and Lift Wellness Center to receive patient referrals. Pathways is integrated into the community with the Lift Wellness Center Disease Management partnership. Lift Wellness Center Disease Management uses a multi-disciplinary team approach to focus on health education including Diabetes, COPD, CHF, and Asthma Education.

While in Jackson, TN, the Account Managers also visited Faith Health Center. This network partner is a medical clinic that provides healthcare, wellness counseling and health education to insured, low income, uninsured, elderly, and disabled patients in Madison county. This clinic also serves several surrounding rural counties of West Tennessee. Faith Health Center is a staple in the community and has been in our network for nearly three years!
