Republic School District Employees,
In preparation of the district receiving a free COVID19 vaccine to distribute to ANY employees who would like to receive it, please complete the form found at the below link if you would like to get the vaccine through the district.
Completing the form will reserve your vaccine. As more details are available in regards to the brand of the vaccine, timeline for distribution and logistics of receiving the vaccine, they will be shared.
The vaccine will be free for all employees, and ANY employee is eligible (full time, part time, etc.). Please complete this form by Friday, January 29 if you'd like to receive a COVID19 vaccine through the district.
