The Bureau of Child Development and Head Start Collaboration is gathering information from programs providing school-age care for children this fall. If this applies to your program, we would very much like to hear from you!

If you have multiple programs, feel free to complete this as many times as you need to. The more we know, the better chance we have of using the data to leverage support and awareness.
 
The information you provide will help us document needs, supplies and issues related to serving children during hours they typically would be in school, if not for the pandemic. Results will also inform our efforts to identify possible solutions to help families access care who otherwise could not afford it. We appreciate your assistance with this survey!
 
Results of the survey will be reported in aggregated form only. We will not share any identifying information without your permission.
