SCC QUARTERLY PROJECT ADVISORY COMMITTEE (PAC) MEETING

On September 26, 2019, SCC held its quarterly Project Advisory Committee (PAC) meeting. The meeting kicked-off with presentations from each of its three Hubs: Catholic Health Services, Northwell Health and Stony Brook Medicine. Each organization highlighted their respective transitional care management programs.  Robert Fortini, PNP, Vice President, Care Coordination, at Catholic Health Services, spoke about their C3 Programs, targeted at high-risk identification criteria/prioritization and improving workflows to influence outcomes. Hallie Bleau, ACNP-BC, CCM, Assistant Vice President, Transitional Care Management, shared Northwell's Health Solutions' strategies to meet the objectives of value-based care delivery and their transitional care management outcomes. Stony Brook Medicine's Care Management Team presented  Community Transition of Care - Highlights of Continuity of Care . They described their initiative to reduce avoidable hospital readmissions through their Together-Strengthening Transitions Avoiding Readmissions (T-Star) Committee.  Stony Brook Medicine's  speakers included: Jere' Freeman, Assistant Director of Operations and Analytics; Mary Ann Lind, Director of Case Management; Susan McCarthy, Director of Social Work and Maura Shovlin, RN Clinical Educator.

Long Island Healthcare Select, Inc., presented  The Value of Virtual Medicine in Creating an Envelope of Care: Lessons Learned with Roll Out and Implementation in a Federally Qualified Health Center.   Lindsay Fudim, MHA, Janet Pepper, MBA, and James Powell, MD, described using telemedicine as a way to bridge access gaps, lower emergency room rates and make it easier for the developmentally disabled patient population to access care. They discussed their ability to access grant funding to help sustain a virtual medicine model.

A performance update was provided by Kevin Bozza, MPA, Chief of Operations, VP Population Health Management Services, SCC.  Mr. Bozza presented Statewide and local performance trends from program inception and performance on specific measures for SCC.   He also gave an overview of the State's measure and attribution changes and how it affects the PPS.

NATIONAL BREAST CANCER AWARENESS MONTH
Other than skin cancer, breast cancer is the most common cancer among American women.  Mammograms are the best way to find breast cancer early, when it is easier to treat and before it is big enough to feel or cause symptoms.  For resources to help educate your communities, visit the Centers for Disease Control and Prevention website.  

MENTAL ILLNESS AWARENESS WEEK
Each year, millions of Americans face the reality of living with a mental health condition. However, mental illness affects everyone directly or indirectly through family, friends or coworkers.   That is why each year, during the first week of October, National Alliance on Mental Illness (NAMI) and participants across the country raise awareness of mental illness. NAMI's website has helpful resources  and statistics  that you may find useful.  

HEALTH LITERACY MONTH
Celebrating 20 years, Health Literacy Month is an annual, worldwide, awareness-raising event that has been going strong since Helen Osborn founded it in 1999.  For resources and to listen to the health literacy podcast, Health Literacy Out Loud, click here

2020 SPECIAL PROJECTS FUND REQUEST FOR PROPOSAL

The New York State Health Foundation (NYSHealth) 2020 Special Projects Fund Request for Proposal (RFP) is now available.   Applicants must submit an online inquiry form to determine whether a proposed project fits the funding criteria. There are two funding cycles for the Special Projects Fund. Deadlines for the online inquiry form are October 3, 2019 and March 4, 2020.  Selected organizations will be invited to submit full proposals, with deadlines of December 2, 2019 and May 11, 2020.

NYSHealth Special Projects Fund awards are one-time, non-renewable funding opportunities consistent with the Foundation's mission to improve the health of all New Yorkers but outside of its main priority areas. NYS MEDICAID POPULATION HEALTH SYMPOSIUM

The NYS Medicaid Population Health Symposium will provide a forum to share the perspectives and insight of the DSRIP Performing Providers Systems, Physical and Behavioral Health Providers, Managed Care Organizations and community organizations addressing the Social Determinants of Health. The goal of the summit is to provide a platform for these perspectives on advancing value-base payment and gain understanding for further discussion.

1115 MEDICAID REDESIGN TEAM WAIVER PUBLIC COMMENT DAY
October 25, 2019

The NYS Department of Health (DOH) will be holding a downstate public comment day for New York's 1115 waiver programs.  The public comment day will be held on October 25, 2019, 11:00 am - 2:00 pm, at Baruch College, 55 Lexington Ave (Corner of 24th Street & Lexington Ave), Room 14-220 (14th Floor), New York, NY.  Attendees are required to check in with the security desk whereupon they will be provided direction to the appropriate room.  Written public comment may be submitted to 1115waivers@health.ny.gov through November 4, 2019.  Please include "1115 Public Forum Comment" in the subject line.

About the 1115 Waiver:
  • The 1115 waiver, also known as the Medicaid Redesign Team (MRT) waiver in New York State, is an agreement between the federal Centers for Medicare and Medicaid Services (CMS) and New York State that allows the State to use a managed care delivery system to deliver benefits to Medicaid members, create efficiencies in the Medicaid program, and enable the extension of coverage to certain individuals who would otherwise be without health insurance.
  • The MRT waiver is a vehicle by which the State has implemented the initiatives of MRT and subsequent health systems reforms, including the Delivery System Reform Incentive Payment (DSRIP) program.
  • New York's goals in implementing its MRT waiver include improving access to health services and achieving better health outcomes for New Yorkers through multiple programs.
This meeting will be webcast live here and will be open to the public.  Registration will begin at 10:30 am, no pre-registration is required.  Individuals who wish to provide comments in person are asked to register on-site no later than 12:00 pm and will speak in the order of their registration.  Comments should be limited to five minutes per presenter to ensure that all public comments can be heard.

NYS DOH ANNOUNCEMENTS

DSRIP WAIVER AMENDMENT CONCEPT PAPER
The NY State Department of Health is providing notice of the State's intent to request approval from the Centers for Medicare and Medicaid Services for a four (4) year waiver amendment to further support quality improvements and cost savings through the Delivery System Reform Incentive Payment (DSRIP) program.  As with the original Medicaid Redesign Team waiver, New York State seeks a continuation of DSRIP for the 1-year balance of the 1115 waiver ending on March 31, 2021, and conceptual agreement to an additional 3 years from April 2021 to March 31, 2024. Thus, the full four-year extension/renewal period (1-year extension and 3 years of renewal) would span from April 1, 2020 through March 31, 2024.   For a draft of the amendment proposal  click here .

LONG TERM CARE PLANNING PROJECT MEETING
The NYS Department of Health and the State Office for the Aging will be holding the fifth meeting of the Long Term Care Planning Project (LTCPP), an initiative designed to understand the projected needs of older adults in New York by examining the State's aging and long term care system.

This meeting of the LTCPP will be held on Friday, October 25, 2019 in Meeting Room 6, on the Concourse of the Empire State Plaza in Albany from 10:00 a.m. - 1:00 p.m. The topic of this meeting will be Financing Alternatives to Public Programs.  At this meeting, participants can expect to hear from speakers on select programs and alternative ways the State should consider financing public programs.

PREVENTIVE PEDIATRIC CARE CLINICAL ADVISORY GROUP RELEASES FINAL REPORT
The NYS Department of Health released the final report of the Preventive Pediatric Care Clinical Advisory Group (CAG), a team of experts in children's healthcare formed under New York State's First 1000 Days on Medicaid Initiative.

The First 1000 Days on Medicaid Initiative is a collaborative effort that recognizes that the first three years of a child's life can have a lasting impact on future health, educational achievement and economic stability.  The report, which is one of ten proposals included in the Initiative, envisions a comprehensive pediatric healthcare model that integrates behavioral health into children's primary care and recommends a two-generational approach to care, focusing on both the biomedical and social-emotional elements that affect early development and are essential to achieving health equity.

The Department would like to acknowledge and thank the CAG co-Chairs, Dr. Mary McCord, Director of Pediatrics at Gouverneur Health, NYC Health + Hospitals and Dr. Dennis Kuo, Associate Professor of Pediatrics at the University of Buffalo, and the Committee Advisors for their dedicated efforts in developing this report.   To learn more about the First 1000 Days on Medicaid Initiative, visit the DOH website .  For a copy of the full report, click here.  

NYS VALUE BASED PAYMENT ROADMAP SUBMISSION TO CMS
The 2019 annual update to the NYS Value Based Payment (VBP) Roadmap (Year 5) has been finalized and the redlined version of the Roadmap has been submitted to the Centers of Medicare and Medicaid Services (CMS).  The updated version of the NYS VBP Roadmap continues to establish the standards and guidelines for the NYS VBP model.  The updated VBP Roadmap may be found on the VBP Resource Library.  Comments or questions may be directed to vbp@health.ny.gov.
