Samuel U. Rodgers Health Center (SURHC) and the Health Care Collaborative (HCC) of Rural Missouri are Working Cooperatively to Transition Ownership of the SURHC Lafayette Family Medicine & Dental to HCC’s Live Well Community Health Centers
Samuel U. Rodgers Family Medicine and Dental, which has served the Lafayette County community for more than a decade, is changing hands. Plans are underway for the Health Care Collaborative (HCC) of Rural Missouri to own and operate the facility. Lexington, Mo.-based HCC currently owns and operates four federally qualified health centers (FQHCs) in Buckner, Carrollton, Concordia, and Waverly. Live Well Community Health Center – Lexington, will be their fifth.
A community forum is planned for Thursday, February 20, 2020 at the Lexington 4-Life Center (811 South Business Highway 13) to discuss the transition and answer questions from the community.
“This process has truly been a collaborative effort,” said HCC CEO Toniann Richard. “We are working lockstep with Samuel U. Rodgers’s CEO Faisal Khan and his transition team to make this a seamless handoff. The community is invited to learn more about where we are in this process, and what this means to Lexington and Lafayette County.”
Richard said this move is backed by and in collaboration with federal, state, and local stakeholders including Health Resources & Services Administration (HRSA), Missouri Primary Care Association, Health Forward Foundation, REACH Healthcare Foundation, among others.
Richard and Khan are working together to make certain the transition is patient friendly, responsive to staff needs and concerns, and properly planned. “We are focused on every detail of this transition,” Richard said. “We want current and prospective patients to know that the positive track record we’ve built with our other Live Well Centers will also be carried out in Lexington. We are excited about the opportunity to serve patients at home where we’ve been headquartered for more than 15 years.”
Although transition efforts are still underway, plans to officially change clinic ownership is slated for late spring or early summer. To learn more about HCC and its Live Well Community Health Centers, visit
