Did you know that up 40% of patients who died of sepsis in the hospital were eligible for—but never referred to—hospice care upon admission?
Our hospice experts encourage hospitals, emergency physicians, specialists and other healthcare professionals to engage in “whole-person” evaluations of seriously ill, sepsis-vulnerable patients when they are admitted to the hospital or ED.

Consulate has a network of 140 post-acute facilities, from Pennsylvania down to Jacksonville, FL. With an emphasis on transitions of care, Consulate is turning to
Collective Medical
to connect these facilities with neighboring hospitals.
"We have always prioritized our patients' needs above all else," says Chris Bryson, CEO of Consulate Health Care. "Our partnership with Collective exemplifies that commitment. We're thrilled to be a part of the movement to collaborate on patients across all points of care."
Similar post-acute facilities have seen measurable results using the platform—like one Oregon-based SNF that cut hospital readmissions by 60% using the platform.

How Our Patient Advocacy Team Supports Patients
Our dedicated Patient Advocacy team provides patients with support and resources during the post-flight billing process. Our goal is to relieve stress on patients and their families, and Patient Advocates have been very successful in helping patients navigate the claims and appeals process.
After the claims and appeals process, the Patient Advocate continues to work with them to ensure that their unique financial situation is considered when insurance leaves them with an unpaid claim. Our average out-of-pocket costs across the country continue to go down and Patient Advocacy will continue to push them lower. In fact, when patients work with our Patient Advocacy team, they rarely pay more than their co-pay and deductible.
